Never factory reset in bootloader on a modded HOX. It corrupts the SD. To fix it, simply connect to a Windows PC, and reformat the SD. Then boot into recovery, mount the SD, and move your ROM of choice to the SD.
In the future, only do factory reset in the OS or recovery.
I'm already helping you with this problem here:

format your sdcard, internal and external from within windows or from recovery, but back it up first from within windows
Format /sdcard wipes your external sdcard.
Format /emmc for your internal sd card.
This is for Sky ICS Build F.
Found this out the hard way when I aoft bricked my phone after format /sdcard and when I went to flash a new rom my external was wiped clean.

The rom (any rom) lives in /system partition ...
/data partition holds your apps
/cache holds your apps data and files (cache files)
/sd card (internal or external) is the place for your personal files
So when doing Factory reset , the recovery will wipe /data + /cache , but your rom would stay untouched like just flashed , the same is applied to the /sd card..
Like Vincom said : connect your phone to PC , from my computer , right click the storage of your phone (after you tap on your phone screen connection) and choose format sure after you backup your important files , then it will be formated or just select everything then delete ... After that boot into recovery then do factory reset ... Your current rom will stay as is ..but everything else is gone ...
Hope this helps.
how does the internal card like being formatted? FAT? FAT32?

Just FYI- In JB(CM10) the new naming convention is /storage/sdcard0 for internal and sdcard1 for external..

Hi all, i have a huge problem. Yesterday i wiped everything in my rom with cwm, wiped data, sd, cache, dalvik and system. I saved on my pc a folder with some files i need to flash and some backups. I would like to mount the usb storage from the recovery, put the files in and flash normally through cwm. BUT, and there is a but, my phone is perfectly view by the pc (it says also Nexus s in periferical) but doesn't see the drive when i mount the usb storage? Do you know why? Is not a problem of drivers, because the phone is perfectly seen. Now i can't boot, because i don't have a SO, and i can't mount the usb storage from cwm. Is there a method to flash roms .zip from fastboot? I'd need the .img file, not the .zip ones. How do i solve?
Thank you very much
I solved by downloading the factory images from google and flashing them from bootloader

i erased everything and tried to do the mount through recovery, and it worked
cant help you
Hi community,
I have my Nexus 5 since 2 Days. Now it cant find the internal storage.
U dont talk about usb, mount at mass storage etc. NO
My Phone didnt recognize the internal storage.
So i can boot it, but can not access.
What i have done:
- its odex stock rom
- open bootloader
- root SuperSU
- Recovery TWRP latest
Until then everything was fine.
Then I tried to mount my USB storage with TWRP.
I know now its not possible..
But I would not give up so I installed CWM latest to try it.
->
fastboot erase cache
fastboot flash recovery cwm.img
fastboot erase cache
After that i navigated to "mount option" and try to mount and unmount all of this buts not working.
So i flashed back to twrp and reboot my phone.
Now the phone has booted, but the internal storage was not recognized.
In addition, there is no GSM / LTE connection.
Because I can not access the storage, I can not f the recovery
So my question:
How does the phone get back access to the memory?
Thank
Try format data and data/media (sdcard) from recovery. If that doesn't work, you may have to flash the factory images from fastboot. In either case, your data will be wiped.

Since you´ve messed up your internal storage, I suggest you to flash a factory image and to hope for the best

What to do?
O wont fail again.
Flashing a factory image is very simple, have a look at eg. this link: http://forum.xda-developers.com/showthread.php?t=2513701

Yeah, thx. Now its stock and full working.
So, yeah. I was trying to get to recovery via Bootloader, and I accidentally went down too far and chose 'Factory Reset'. How can I fix this? Can't mount to format my SD Card, and can't look at any pictures, download things, save downloads etc..
Your data is gone, the factory reset in bootloader has corrupted the sd card. You need to mount the sd card in TWRP while connected to your PC and format it using Windows.

Please, help me, i have an LG P920 OPTIMUS, and i wipe factory reset using clockwork, and after the reset this is what i get LG OPTIMUS, not enough free space clean up your data and try again. I deleted everything in the sd cards and formatted them, still i get the error.. I can't download from the market thing, and i can't receive or send text because the storage is full, when it is not. please help
Reflash it
Hi!
What is your android system?
Try to reflash the system, when it finished, power on, let it boot to system, click exit on setup wizard, restart to recovery and do a factory reset.
After it done, do a reboot, and your storage now working.
It worked for me in official GB 21E.
On stock GB Roms, don't do a factory reset with CWM installed, it will keep giving that error. Flash stock recovery on your phone and do another factory reset. That always gets the job done for me.
try this:
1. first check in settings - app manager - how much is the total size of internal storage and SD card (sometimes corrupt installation can access only little of these space). then backup whatever you want to on the computer . download and put a rom.zip in external SD card.
2. if you have root access then boot into recovery , wipe delvik cache , wipe cache , factory reset and wipe system , format internal storage this will completely remove the ROM installed and all the apps. then reboot into recovery and flash the rom.zip from sd card.
reboot and enjoy!
