Both times I have wiped data/factory reset with the latest version of Clockworkmod, the recovery has not been able to mount my system partition. The first time this happened, I put the phone into fastboot and flashed TWRP. It couldn't mount the system either.
After the first time, I thought it was a fluke. However, I just wiped data/factory reset again yesterday and the same thing happened. I had to flash the restore all binaries from Google to get the phone back up.
So, my question is, why is this happening? I'm far from new when it comes to root/custom ROMs, and I've never had this issue before on any of my previous phones (EVO, Nexus S, GS3). Does anyone have any idea why this would be corrupting the system partition? I've flashed TWRP to see if that performs any better.
It could be the emmc going bad. Leo an eye on it. In the future, instead of flashing the flash-all.bat just flash the cache.img in fastboot

That would be odd that it'd be going bad so soon. I've only had the phone about 2 months, and I've not flashed much besides a few ROMs and kernels.

I have a T-mobile G2 android phone. I recently rooted the phone. Installed different roms, and attempted to install a different rom. Currently have ClockworkMod Recovery v2.5.1.3 installed. After restarting phone, if I don't load the ClockworkMod, the phone will freeze at the "HTC" screen. I can get into the Bootloader or into the ClockworkMod, but cannot get beyond that. What's the easiest thing to do in this situation?
Wipe data and cache and reflash either your ROM, or a Nand backup. That happened to me trying to ROM my Nook Color's emmc. If wiping data and cache doesn't help, you'll need to format data, cache, boot, and system and reflash. (Hopefully you've backed up)
Note: It's most likely your boot that's a little wonky, not CWM. IMHO CWM is one of the best things to ever happen to us Droid geeks
Ok honestly, Clockwork doesn't work on my Dream 32a. It doesn't boot up into the recovery. It's not compatible with some phones. Hopefully you did a nand backup..
I didn't do NAND backup. Is there an alternative.
Reflash whatever ROM you want, but format the 4 listed above. It's your boot that's messed up, so overwriting it with another one will confuse it... My nook boot-looped for about two weeks straight when I first tried installing to it's internal memory. I just stuck to it and read the forums over and over and never lost hope or gave up.

I have the Telstra HTC One XL, and have just recently attempted to install CM10 ROM - however have run into some problems that I would appreciate some advice to rectify. I had recently updated to Jelly Bean OTA.

I Successfully installed TWRP Custom Recovery
I Downloaded CM10 as per - http://forum.xda-developers.com/showthread.php?t=1790032. I transferred this as well as Google Apps onto my phones storage.
When attempting to install CM10, I created the Nandroid backup as recommended, and proceeded to wipe the mentioned items using TWRP (a guide I saw said to wipe everything). This is when my Nandroid back-up got wiped. TWRP gives you the option to wipe cache, delvik cache, factory reset, system, external storage, internal storage and format data. I did alll of these, and this was my mistake I believe? I proceeded to try and install CM10 and gapps from internal storage (I had to re load these back on after I wiped everything), and the install of CM10 displayed as successful.
Upon rebooting to system, the HTC screen comes up first and then goes to the CM10 animation. The phone then stays on this animation (rotating circle) for up to 10-15 minutes.
I have read thread after thread with people experiencing similar issues, and have tried rectifying my problem with various versions of roms and different guides out there. I can still boot into recovery (TWRP) mode via bootloader, and this means I can still mount my phone's storage onto my PC and have access to the storage directory. As I have deleted the Nandroid backup though, and because the ROM's won't install, I am stuck in no man's land.
There must surely be a way to have a ROM installed (even if its back to stock Telstra) from the position I am in, it would be greatly appreciated if someone may be able to point me in the right direction.

Why do people keep doing this??? Just because TWRP gives you all those options, doesn't mean you should use them.
Sometimes, people will say to do a "full wipe" when flashing a ROM. But that term is a bit misleading. Factory reset (from TWRP, never from hboot), wipe Dalvik and wipe cache is all that is technically required when flashing a new ROM. Some people like to wipe more "just in case". But if you aren't completely confidant in what you are doing, keep in simple and don't make it more complicated than it needs to be.
I don't know if wiping everything is the cause of your woes, but it certainly didn't help, and caused you to do some extra steps.
Flash boot.img via fastboot as previously suggested. Otherwise, just try to re-download and flash the files agin. Its seems that some people are having troubles booting after flashing CM10 (even after doing the correct wipes and flashing boot.img). Doesn't seem to be much rhyme or reason, but just flashing again has fixed the problem for at least one person.

I have installed cm-10-20130107-NIGHTLY-evita and cm-10.0.0-evita , however with both of these versions, my touch screen is not working once it boots up. I can function with the volume up/down, and with the power buttons, but cannot get past my SIM card lock screen.

Does anyone have a solution to this issue that seems a common one?
Alternatively, more than happy to get an alternative ROM on my phone in the interim...Can someone recommend me a ROM?

Last night I flash a custom kernel and then decided to wipe cache/davik but it froze mid-way, so I did a hard reboot back into recovery and to my surprise I can't mount cache and usb-otg and to make things worse, the sdcard is not recognized. I can boot into the rom but sdcard is still not recognized and there is no cellular service. Question is there anyway for me to fix this, I have data on the phone that is very important to me.

Fastboot flash the cache.img from the factory image
somebody made a guide for getting your data back and fixing this issue on the nexus 5. It was posted in the n5 general section a day or so back.
I got a nexus 5 off of eBay, the seller said he had tried to root it and failed so I thought I would have a crack at it.
I thought it would be an easy restore but I ran into a few problems:
Phone originally came with 6.0.1 installed.
On android 5+ I could not access the standard recovery screen, and when I pressed power+volUp it just said formatting /data and would not move from that.
Rolled back to 4.4 and I got into the ususal recovery screen.
Tried to do a factory reset, but it again stuck at formatting /data.
I left it the entire night while I slept and it was still saying formatting /data. in the morning, nothing had changed.
Trying to boot the phone normally just left it on the 4 startup screen.
I've tried doing a full flash of stock android with each seperate part manually.
Also /data is unable to be mounted in any custom recovery, like it is corrupted or locked somehow.
With the stock recovery, it cannot be detected by adb, but in cwm it can.
EDIT: checking the /data folder in adb shell shows it's completely empty, and the same with system even after flashing and flashing again.
Can anyone advise me as to what might be the problem here?
Reflash all partitions manually, including userdata.img file. After flashing the last partition, boot directly into stock recovery and perform a factory wipe, then reboot.
Done that multiple times, I get the same result on stock android.
I don't want to root or anything like that right now, just get it back to a working state.

If you flashed userdata.img multiple times and the phone still can't perform a factory reset, the emmc chip may be damaged.

So I have a few dozen Moto Gs. I'm always running into problems with them, but this problem is new. I had originally used CWM on this device for backups and rooting. Phone died and a put it in the drawer for months. I took it back out to mess around with it and see if I could get it going again. However, I can't get TWRP onto it. I've tried erasing recovery via fastboot, it says it worked, but when i boot into recovery it's still CWM. Inside CWM itself, I get lots of "can't mount cache" errors. So when I try to wipe anything inside CWM, it just freezes.
I was able to boot directly to TWRP via "fastboot boot twrp.img". However, none of the wipe tools seem to do anything. Factory reset, format data, advanced wipe... they all go through with no errors, but the file manager still shows that all my original files are still on the phone.
i have same issue with xt1033 but instead of cwm mine is stock recovery no matter what i do with my device everything stays same seems like it went into write protection have u got any fix?
Oh dear - it looks you two have suffered from the read-only eMMC brick. You can try repairing the phone with this tutorial: https://forum.xda-developers.com/moto-g/help/how-to-unbrick-moto-g-falcon-t3394788
If this fails though, your phone needs to have its logic board/motherboard replaced. That said, it does make me want to ask one question: for those who have suffered from the read-only eMMC brick, what eMMC chip do those who have suffered from this bug have? Is it a Samsung or a SanDisk eMMC chip? I think it is about time to do a survey here.
