This is my latest theme for the latest CyanogenMOD ROMS by Wolfbreak & Rendeiro. Currently I have built this on Wolfbreak TW 007 with NO problems

Installation instructions:
1. Download Theme v1.1] [Xrecovery] - http://www.mediafire.com/?9itcbk2ike0qyqz [Please dont mirror these downloads]
Again be sure you have the latest ROM TK 007 from Wolfbreak or Rendeiro's CM6.1.3 ROM [Read Above]
[optional]Download Extras Pack [See Screenshots] - http://www.mediafire.com/?9cxoonud1xt5pk4 [Please dont mirror these downloads]
2. Extract to SD Card then boot into xRecovery, select Install Custom Zip, Select from SDCard then select the Theme zip file you downloaded...reboot....Enjoy
[Optional] If you downloaded the Extras pack mess around with the Icons / Dock bars & Wallpapers and have fun!! To set custom icons use ADW/Launcher pro to make custom shortcuts

1.download the theme autumn style for creed rom 1.5
2.copy to ur SD Card /sdcard (should not in any folder)
3.reboot to recovery CWM
4.install from sd card

2.In Gravity Box make status and navigation bar Transperant - Xposed installer > Modules > Gravity Box > Transperency management and make everything to 100% and Reboot
2.1.Xposed installer > Modules > Gravity Box > StatusBar Tweaks > StatusBar colors and tick "Enable icon colors". Click Statusbar icon color and make them white.
2.2.Xposed installer > Modules > Gravity Box > StatusBar Tweaks > Battery settings click "Battery indicator style" and tick "KitKat Battery"
2.3..Xposed installer > Modules > Gravity Box > StatusBar Tweaks > Battery settings tick "Battery percent text"
3.With Theme Chooser choose CM11 CM10.2 TouchWiz 5.0 theme
4.Make S launcher default
4.1.In S setting make Homescreen grid 5x4 and App drawer grid 4x4
5.Put weather widget 5x2 on Homescreen and make a settings to your liking
6.In galaxy lockscreen setting tick "Enable Galaxy Locker"
6.1.Lockscreen Effect - Ripple effect
6.2.Ink color - None
6.3.Tick "Shortcuts"
6.4.Hide Statusbar
7.Hit Thank's button

PS: For an even more TouchWiz-like status bar, download the Tinted Status Bar Xposed module and set the status bar color for S Launcher to be 66000000.

I guess Deodexed ROM is required for Xposed.
And I don't find it mentioned in the OP.

ok so first install external launcher the one you whant on playstore
copy your old launcher to your sd card (just in case) and delete it if you have my magazine you can delete it also
download the version you whant 4x5 or 5x5 and copy it to system/priv-app and set correct perm if your rom is odex so remove touchwiz.odex
download wallpapercropper and wallpaperpicker and paste it to system/priv-app , remove wallpaperchooser.apk
download the apk secthemechooser + flipboard + flipboard briefing and the theme you whant and copy it to system/app and set correct perm
reboot your phone and start with external launcher go to setting application and go to the last tab you can see touchwiz so clik on it and supress data
push the home button to start touchwiz
