So I was going to take a nandroid of my Nexus 5 in order to transfer to my RMA device. I have found that device swaps usually go over better using CWM for nandroid instead of TWRP. So I flashed the latest CWM from ROM Manager and booted into it.
I figured I would save space if I blanked the cache, so I initiated a cache wipe. The display become a broken-up Android picture, and the cache wipe did not complete in minutes. I long pressed power to boot into the OS and found that - uh oh - the entire sdcard folder was inaccessible. All app data appeared to be intact but no app could read or write the sd.
Did I just completely bork all my internal storage? Any possible rescue path?
EDIT: OK I flashed recovery over to TWRP and its file manager still shows all my files. So all is not lost. But something went wonky and the OS is confused now.
In TWRP when I try a cache wipe I get:
Updating partition details...
E:unable to mount '/cache'
E:unable to mount '/cache'
E:unable to mount '/cache'
Formatting Cache using make_ext4fs function.
And it sits there chugging the status bar. Been over a minute now.
Aha okay, it finished. Fingers crossed... Yeah OK it's fine now on another full boot.
Still a bit strange though. Normally on any other device I've owned, a cache wipe was a simple matter.

Its an issue on the nexus 5 for some reason, took 5-10 min on my phone before but i think it was fixed in the latest twrp.

Yes, on TWRP 2.6.3.4 it took 5-10 minutes but fixed it. It would seem that there is a simple cache wipe versus fully formatting the partition. If the first fails to work, it proceeds to the full format.

I am trying to update my xoom to a new rom, ICS, and yet when I go into recovery to wipe data/cache, nothing gets touched by this command. I want to wipe the xoom clean and start over yet I can never get this command to work. I want to format the internal storage and start fresh yet every time I try this command all of the folders and apps and data are still left on the internal storage.
Any suggestions on how to get this fixed? I wanted to just mount the storage while in recovery and then perform a format on the device from within windows yet I can never get the device to mount from recovery. The only thing I can think of is to re-root/unlock the device and fastboot flash a new image to the device. Any thoughts?

Sometimes you have to mount /data and /system before those commands work. For some reason I see those file systems don't get mounted when cwm or twr loads.

Hi Everyone,
For some reason, TWRP is not able to format /Data.
It keeps saying "failed to mount /data (Device or resource busy)".
This doesn't prevent me from updating the ROM to 4.4.2-21.
However, once updated it keeps on booting over-and-over.
Is there a way to - sort-of - a forced format of the internal flash.
Or at least the /data partition as it seems to be corrept.
Thanks - Will
Hi,
With twrp use the wipe menu for installing kk 4.4.2
Wipe cache-dalvik cache-data-system-internal storage (if you need a complete clean install), after install menu, flash the rom and the gapps, return to wipe menu, wipe cache and dalvik cache again.reboot.you`ll run on the new rom
(Ps : don't used mount sd and format menu).

Thanks for the (quick) response.
I'm coming from 4.4.2-19. However, on previous occasions I was able to upgrade without going through a clean install.
So I tried the same this time with 4.4.2-21. However, as it stands, this release tends to end up in an ever on-going boot sequence.
Even a clean install or going back to the previous version (4.4.2-19) doesn't work.
Compared to previous installs, the only difference I can see is an error stating that /data can not be wiped.
Any thoughts on this behavior?
Thanks - Will

Hi, which release of TWRP do you use ? in 2.6.3.0 and lower release, on wipe menu you have an advanced wipe menu, go on it select format and type data, this will format data partition

Tried formatting the data partition. It comes back with "failed" - no explanation.
Any other suggestion?
This is a really weird problem that i have come across.
I had crombi-kk installed in it. But because of some bugs, i decided to install cm10.2.
I booted up in twrp v2.7.1.1 recovery and chose to Wipe>format data. It started but never ended.
When about 20mins had passed it was still the same, i thought it had hanged so i rebooted.
Then i sideloaded cm10.2. After twrp installed the rom, it showed an error E: unable to mount /data.
I rebooted and it didnot go any further from cyanogenmod bootanimation.
I again rebooted into recovery and chose Factory Default, it failed and showed the above error and 1 more E: unable to mount internal memory.
Then I flashed cwm v6.0.4.7 and i sideloaded which resulted the same. Then i went to mounts and storage and chose mount usb.
But it didnot show up in my pc.
I searched over the net and found that this kind of problem occured when the sdcard was mounted as logical.
Ok, i get that, but now how do i fix this problem? The sdcard isn't showing up on the pc!!

It takes about 2 hours to format data so you interrupted it in the middle.
You need to format the data again and this time let it finish please.

Are you sure this would really work?
because it shows, E: Can't mount /data
E: Can't mount internal memory

2 hours? Really? What does it do?
It just wipes the days, right.
Now don't say it recreates the partition

No TWRP changed it a few versions back I think it fills every single piece of the emmc with 0000s. Which on mine took 90mins.
Reflash TWRP and see if it will let you do it again. Otherwise you may have to erase it from fastboot.

Okay. I haven't seen the source. But shouldn't be a problem on trwp 2.6.x, right?
And why did they change it to fill the whole block with 0's?

That was a change in the base Android libraries. It actually doesn't fill the device with zeros, it calls the "secure erase" command of the eMMC (aka the secure version of "trim"), and that is slow.
Pardon me but i cannot find the right place where to post my problem.
After failing horribly to instal zombie over crombi and failing also to restore a nandroid, I tried to do a factory format but now it says unable to mount /cache twfunc::copy logcant open dest log file /cachemkfs.f2fs binary not found, using rm -rf to wipe
wiping data without wiping /data/media... Done.updating part details...
E"unable to mount ,' /cache
I really feel at a loss as when it happened the last time on the other tablet it came round fine after a full format, but this one doesn't want to know..
Please help thanks

Hey mate ....
Sounds like you got yourself into a pickle .......
Give this a try .... Boot into recovery and ........
Code:
adb shell
killall -19 recovery
make_ext4fs /dev/block/mmcblk0p8
This should format /data, You will need to use the hardware button to force reboot......
reboot back into recovery and see if /data can be mounted now ......
Thx Josh
Thanks for the quick reply do you think this will fix cache mount and dalvik mount problems as the system can wipe date but not dalvik and cache.
Fixed

After much formats it appears to be that twrp was corrupt as after using a fresh recovery blob every thing seems to be in place and cache and dalviki can be mounted.
The only quirk is that no matter how I try to instal Zombie after the message to restart and enjoy it just hangs on the droid animation for more than an hour, so for the time being I went back to crombi-kk until someone knocks some ideas into me.
Thanks

Are you on the 10.6.1.14.10 bootloader?
Yes exactly that. Thanks
at first im just failing at wiping cache.
now i cant even restore my TWRP backup.
it is saying "Failed to unmount '/cache' (Device or resource busy)
now my phone got no OS :crying::crying::crying:
I was facing the same problem.recovery twrp>wipe>advance wipe and then select system>repare or change size>change file system>select FAT
after doing it try to restore your backup and if you get error the only way flash your phone to the last build number(avalaible for your phone) and then install twrp recovery(last version).
restoring backup should be work with this method.
you can try this method
Mount>select partitions to mount : cache
select cache and after doing it go for restoring.

No need to restore /cache partition, exclude it The partition is used by TWRP that's why you can't unmount it.
I still have the same problem persisted. Can't wipe cache with this TWRP recovery. It says can't mount/unmont cache device or resource busy. Restore of the backup was impossible as well. Even when cache was excluded. Changing file types and repairing it also didn' work. In Mount cache is ticked permanently and it can't be unticked.
