It is happening to me on all ASOP builds using BT voice dialing. I speak a command and there is up to a 5 second delay in responding. This has been a problem on most of these build for at least a month.

I've been having an issue with Bluetooth streaming audio on every original development rom I've flashed. I'm currently running Slim-4.2.2 build 5.
The problem is that when I'm listening to music through my bluetooth headphones (LG Tone HBS-700) and an incoming call comes through, the music I'm listening to through my headphones comes blaring out the phone's speaker instead of being paused. This happens only when the phone is in vibrate or silent mode. If I have the phone in ringer mode, then the music will pause and the actual ring tone will play out the phone's speaker as it should.
I use Poweramp for local music and Pandora and Spotify as well. For some reason the problem doesn't show itself when I'm listening to Poweramp; only when I'm listening to Pandora or Spotify. (I haven't tried any other apps).
It's only incoming calls too, notifications don't play out the phone's speaker while in silent/vibrate mode during music playback.
A temporary solution for me is to set my ringtone to "none", but then when I have my phone in ringer mode, I have to remember to manually set the ringtone to the one I want and vice versa. I could use something like tasker to handle that, but I'd prefer to fix the root problem if possible vs. covering it up with a hack.
Has anyone else encountered this and hopefully been able to fix it? I'd really like to leave my phone in vibrate mode at work without the possibility of my music blaring out the phone if I get a phone call.
Not a single person has encountered this issue?

Have you tried an app called Soundabout? It is not just a bluetooth issue even with wired headsets this happens also. You will have to get the paid version for Bluetooth settings.
Yeah, I tried out Soundabout with the pro license. I messed with every setting and I couldn't get it to fix the problem.
I never use wired headphones so I didn't know that was an issue too. I just tried out some wired headphones to see if it behaved the same way and there's actually zero issue with wired headphones in relation to an incoming call on my phone. In silent mode, the audio continues to play out the wired headphones and I see a phone call on my screen.
As it turns out, now setting my ringtone to "None" doesn't work as a temporary solution anymore. Media audio from Pandora and Spotify will reroute to the phone's speaker for an incoming call on silent or vibrate mode. I'm going to try out Slim's weekly build 5.5. If that doesn't work, I'm going to check out a custom kernel. If nothing, I may need to revert back to stock as this is a deal-breaker. I can't have my phone making unnecessary noise in a quiet area in the slim chance I get a phone call.
Any and all solutions are welcome if anyone has any ideas or troubleshooting methods they'd like to suggest. Thanks!

i am getting the same thing and running a stock Droid Razr android v. 4.1.2
very f'n annoying when i get calls at work and i got jams playing and all the sudden just blares out the phone speakers.

Having the same issue with LG Tone+ using PlayerPro with local music. I don't know if it makes a difference (I guess I could test it), but I use last.fm to scrobble all music live.

I think this question has been talked about in another threads/forums.
but I can't find any thread that matches my problem.
To my problem:
When I listen to spotify(using bluetooth headset Sony SBH50) and receive a notification or use TTS, the bluetooth sound get maxed out.
I can't lower the sound until the notification//TTS sound is finished.
It started after last update of spotify
TS done:
Tested another headset (MW600) and I get the same problem, just it has it's own sound settings so I don't get deaf.
If I don't use BT headset I see that the sound get bumped up abit too but not maxed.
Inactivated tasker, same problem.
I have resettet the SBH50 headset and updated firmware.
Tested a mp3 file in xiialive and total commander(music player).
The music get a bit muted during the notification and then back to the same volume level. So sound is not maxed.
Tested Audible and it has a pause function on notification but no maxed out volume.
Solution / workaround
Solution: ? or to get Spotify fix the problem if it's a bug in the app.
Workaround: Was to change settings files to hard set the max BT volume to 50% of what is max today. If it can be done.
System:
Nexus 5
Android 4.4.2
Factory ROM
Rooted
Uses Tasker, don't have any sound alterations tasks.
Used xposed and GravityBox but exposed is deactivated.

Same problem. Extremely annoying. Imagine the situation, casually using bluetooth earphones while shopping, running, commuting & whatnot. And suddenly the music volume maxes out because a notification eg. for a hangout msg or sms goes off. Any ideas how to cure? Re-route bt audio?
Here's something to try:
with the BT headset connected, go to Settings -> Sound -> Volumes and change the notification volume there (again, WITH headset connected)
This should then set the notification volume for the BT headset without changing the media volume.
I use a BT headset all the time with A2DP (LG Tone+) and have never had this problem because I set the individual volume levels as force of habit the first time I connected to N5...

To test it it's best to put very low volume using headphones and whenever sound is playing there is this hiss / noise.. when sound/music stop the noise also stops after 2-3 seconds or so.. And it happens only with headphones, no problem with speakers..
I tried 3 different headphones, some have higher and some lower noise but i can notice on all..

Anyone else having issues with their headphone jack regularly pausing music? It's not the headphones because they work fine in other devices, and different sets of headphones cause the problem. It's not a Spotify problem because Poweramp causes it too. It doesn't seem to be caused by moving the headphones, because I can wiggle the wires as well as the end of the plug and it doesn't cause it. It seems straight up random and it's driving me crazy.
Not that it'd matter, but stock, unrooted, locker bootloader, Verizon Pixel 128GB.
I think it is the notification system causing audio ducking. Try "vibrate only" for notification/ringtone volume when listening to music on headphones. That's worked for me.

That can't be it because I'm usually always on vibrate or silent, and it still happens on airplane mode with no notifications coming in.
Hey I think this problem came after updating to official pie.
The problem is that if I play music on Spotify using a Bluetooth speaker the volume sometimes lowers itself then goes back to the normal volume, then lowers itself again, and it keeps doing that. I have tried 2 different Bluetooth speakers and same problem on both of them. This doesn't happen to my friends phone on of the speakers which is his. Does anyone else have this problem ask as well? It's have you fixed it? Headphone volume is working properly, it's only the Bluetooth.
I think the problem was that I had the volume slider on my phone at Max and only used the buttons on the speaker device to adjust the volume. Having the slider at about 50% I now don't have that problem, for now at least. It was the low bassy parts of songs that most often got it's volume lowered for bit.
You're not crazy! It happened to me and drove me crazy. I think it was an issue with Spotify although I'm not sure but it's gone for me now as far as I can tell..
Finally someone else also. I posted about this one other time but nobody came across. But I guess V4A helped in it, I'm not sure though.

Drove me crazy as well, I did a factory reset. However, the problem got worse, the maximum volume of all my bluetooth devices was extremely low.
I finally fixed the problem by disabling the "Disable absolute volume" in the developer option menu.
Love the oneplus phones, (started at the one), but the audio especially over bluetooth really bothers me...

My first OnePlus phone was also the One. And upgrading to the 6 best decision ever still. But yeah the Bluetooth problem needs to be fixed :/ I tried the disable absolute volume to a while ago but it didn't help for me with the volume going up an down by itself.
This is happening to me. I thought it was my headphones or Deezer but I've tried the headphones with more devices and it doesn't happen. Glad I'm not the only one.
I've had this same issue occur whenever I had V4A Installed. After installation and setup, it'd work just fine, then randomly this same issue occurred. Curious to see if anyone can relate
same problem here, I fixed it by using a custom rom for now
Turn off Bluetooth
Connect a Wired headset go to Audio tuner
Disable Equalizer
Disconnect your wired headset
Turn of Bluetooth again
And Valla! Problem solved.

Same, but it happened right after 9.0.3 I was trying to listen to some music on YouTube and SoundCloud. It doesn't matter what app I think

This fix is working great. Somehow the audio tuner was conflicting with Bluetooth audio output. Although I feel the sound is now a bit flat, the annoying issue is resolved. Need to test this on 9.0.3.

After updating to 9.0.3 Oxygen OS. The issue is completely gone. We can now use audio tuner on Bluetooth headphones.
