My phone was on clean rom 5.0, Loved it. Everything worked great for 2 months. 3 days ago I flashed CM10. Worked alright. Then flashed MIUI (pre-most recent update). Also worked fine except for some screen flickering. Decided to go back to stock. Did a nandroid restore and when it was finished I had no data or bars period. Restarted the phone to no avail. Called Verizon. They assumed my simm card was fried. Then I went and got a new simm card. They tried to activate it to no avail. After changing my PRL from 00000 to 15154 they were able to get 4g on my phone. It's a little slow but it works. Still no calls and text.
My phone number is 0000004548. Any ideas on fixing this?

I completed the guide in the first post. No dice. I got everything sorted out, My phone number is now correct and all but... I cannot get signal still and I cannot boot into recovery. Weird. Any advice would be great! I'm sure others are in my spot.
Is your imei intact? I also had this problem prior to flashing miui. What I did was go back to completely stock, there is a guide somewhere here, make sure you backup all your data. Once you've boot up to stock, you have to dial the re provision code which is *2767*3855# , it will wipe all your data I believe, then reboot it should work then. After that just reroot unlock flash roms. I hope it works for you. If you've lost your imei you have to do something else which a guide is written on here, but I'm pretty sure you haven't lost your imei.

Yes, first thing to do is to check if your IMEI is intact. Also check your ERI (it should be set to 5).
Just a heads up, the re-provision code @Watoy mentioned only works on early build stock ICS. You can use the code in Root66, so my advice is to Odin back to Root66, which is available for download from this thread:

Alright. Flashed stock.vzw_root66.tar and it bootlooped. I pulled the battery after ten minutes and went into recovery and f/d reset. Network unknown. Signal strength and network type is the same. now service state says in service. Mobile network state still disconnected. PRL changed to 15173. phone number is correct. MIN is 3194421067. ERI is still 5. MEID now reads DEC: 256691456608999046 HEX: 0x99000206895086. IP address is now unavailable.
Can you read all that?
Okay I got the reprovision number to work. Going through that now.
I FIXED IT, Thanks to ya'll.

Really? That's not too bad of a price. I thought the screens were going for close to that on ebay, and that's without installation obviously.

A quick look at the thread you linked to, it looks like you need to be back on an ICS ROM (at least for TW) to use the factory reset/reprovision(?) code, after repairing the IMEI.

You didn't miss anything, you just need to do that re-provision code. Flash back to this, do the re-provision, and you will be back to normal. No other way about it unfortunately.

This just saved me! Thanks a ton, sorry about your phone though. Just fixing the IMEI and ERI didn't work for me, I had to reset with the code

I suffered from no sms and no calls, reflashed root_66 tar from odin, then reprovisioned with *2767*3855# and i got sms and calls back but was stuck on roaming with it always trying to get me to install dual clocks, i figured it was a sim card issue as my imei was proper and everything was as stock, i took it to local Verizon, they installed new SIM for free, but it did not fix the issue. With a few kind words and pointing out the extras i spent on accesories, i ended up getting into the Note II. Keep In Mind i was still in my 30 day worry free, but for the SIM card it didnt matter, he did not look up that info till he told me he was going to replace my device if the SIM didnt take. I STRONGLY advise to approach them with kindness, i also informed them it happened when i kept toggling connection when transfering media thru the usb cable, i figured the heat and constant connect/disconnect fried the sim, they never asked if i tried to download any off market apps or anything along the MOD lines. Hope this helps anybody that may need it.. i did follow step 5 of the guide everyone references about rooting jellybean(root,custom recovery,stock), sorry new to forums and havnt figured out the linking yet, but once on the stock file with this name,( COMBINATION_I535VRBLK3_I535VZWBLK3_381038_REV09_user_low_ship.tar)everything read stock except my binary count, but i heard this is only an issue if sending to samsung as verizon reps are unaware of binary counts and odin and stuff related

i was on gummycharged ep1f/ep1h rom and everything was working fine until today.
i don't know what triggered it, but my phone just restarted and then lost all signal.
after searching and looking at phone info, i found first line under baseband version shows "unknown".
tried flashing to every possible roms out there using odin and it would not bring the phone back to life.
funny thing is i have no problem w/ 4glte service, which allows me to surf and download but no voice service.....
do i have no choice but to go to verizon for replacement phone?

yeah I'd take it into them, sounds like some hardware got fried.. however I'd probably try to flash a froyo rom for whatever good it'd do, if they somehow can check baseband and it shows GB your warranty is void and they will not fix or replace it

Had the same problem with my Fascinate. After a flash I lost all 3G and phone capabilities. What I did was reset my phone line with the esn of an old phone I had lying around. Go to this thread. It will show you what you need to do.http://forum.xda-developers.com/showthread.php?t=1051543. I took mine to Verizon, and also spent hours with tech support and, to no avail. This esn swap did the trick. Good luck.

yes, all the roms i flashed had full radio from ED1, ED2, EE4, EP1F

i called Verizon and had them do it over the phone. In other words, they moved my line to a dummy ESN on their end and some random number to my phone and then switched it right back, all while my phone was off, so when my phone was turned back on it got re provisioned on their network, which fixed my 3G data issues......but i dont think this is the OP's solution. When i had that problem I had phone service, just intermittent or no 3g data is all

You won't lose your unlimited data unless you switch to an ESN that is not a smartphone. I have switched my line a couple times and still have unlimited.

I am not sure abotu this (not saying it is wrong but I never tried) I say take the 20 minutes on the phone with VZW and let them do it...
UPDATE
took it to verizon store twice, got sim cards replaced twice at two different locations.
still no go, rep recommended me to contact replacement center.
talk to tech for and hour trying to figure it out,
the tech announced that my cdma radio is fried.....
so i asked when i'll get the replacement and he said sometime on tuesday.
and i asked if i could use my really old LG non-smart phone for time being and he said i'll loose my unlimited data and moved to 2gb tiered data plan...
so i guess i'll be out of service until tuesday....

Hey I believe ya, I am just in the better safe than sorry frame of mind when it comes to the unlimited data structure. It does make sense though since i did switch from the fascinate to the charge without losing it
they told me this will only work if you went from smart phone to another smart phone.
however, if you went to non smart phone and then smart phone, you'd lose your unlimited plan.
I'm surprised this couldn't be fixed with a software flash. I came from a Samsung Epic and this was a regular occurrence on at least one of the Gingerbread leaks. Happened to me at least once - used ODIN and reflashed the whole shebang (full TAR with modem) and got it back up and running.
i wish there was an easy fix like you said.
then i don't have to wait for them to send me replacement device and also saves me from hassle of reapplying invisible shield. the maximum protection kit is pain in the ass to apply.
i've flashed with every single rom+radio out there in many different order with both odin 1.3/1.7/1.8 and samsung pstlite.
i've also tried flashing radio only and still no luck.
Some background. First I'm an idiot for not backing up my IMEI. I didn't want to bother with a simple copy/paste/run some script, and I took my chances (since I already did a successful flash). I had to inject my IMEI
Also, I looked up this issue and while others have talked about it briefly in the dev forum, no one's had a good answer. I dug for a good 2 hours.
So - after flashing a CM10 nightly, of course I lost my IMEI. I still had full 4G service though.
Since I didn't have a backup, I had to do an injection from CM10. So I ran the script to get my USB working on my computer, got in, replaced my IMIE and verified on my phone that yes, I had typed it in right.
I Still have 4G. Texts come in, but I cannot send out. My MEID reads 00000000000000 (14 zeroes). My phone number reads 000-000-0016 (I assure you this is not my phone number). And of course, I have no voice. All I have is an empty right triangle next to my battery indicator.
Is there anything I can do? I don't know what my MEID was, and I hope I'm not screwed. My IMEI is correct, and displayed on the phone.
Any help is much appreciated.
**EDIT** - When I go into the settings, IMEI is correct. However, when I go into phone info, IMEI is B0000000. If that helps. I'm going to return to the forums to see if I can find a solution.
That's normal. Try updating to a newer nightly. It should go away, this is not a permanent problem

I switched back to my old DINC because I need a phone with voice. Will reactivation be an issue at all?
EDIT: So I learned that the MEID is the same thing as the IMEI formatted differently (0xIMEI minus last digit - in hex).
So i am to understand that if the MEID is all zeroes that its not an issue, and it'll work itself out on its own?

I was running CleanRom 4.5 and installed the 4.5 keyboard and clock and all of a sudden I lost my network connection. what is weird is the 4G works fine. I just can't receive or make calls/text messages. Does anyone have any idea what may be the problem? I've returned my phone to stock, cause I'm thinking I'm going to have to take it back to Verizon. Just wondering if maybe anyone knew a fix for this.

Yeah, i dont know what caused it to happen. I didn't even really notice it wasn't working, cause I never lost data. i've always had 4G signal. i just can't make phone calls or send texts. at one point it did say mobile network not available, but now it just doesn't make calls at all. the screen looks like its making the call, but you do not hear any dial tone or anything. For sure it was working yesterday because i was on a four hour call last night. Just stopped out of nowhere today.
I had the same issue the other day. Did you backup your IMEI/MEID info? Check and see if they are still there in about phone, also check your ERI and if your network is listed.

I just got back from the Verizon store after spending 2 hours there. It did seem like it was an IMEI issue. They mentioned my phone having a different number than what was on the phone itself and they could find no way to correct it. The verizon tech support that they called were insisting that it was a network issue and not a phone issue, so they opened a trouble ticket to try and resolve that. they said 24-72 hours to get it resolved. luckily, the store clerks and managers thought support was full of it, so they decided to overnight me a replacement phone anyways just in case, even though they were told not to. so hopefully the new phone fixes the issue. I'm just glad that they helped me out because I didn't buy warranty or anything on the phone so they probably could have flat out denied me. Fingers crossed that all is good in a couple of days
I did back up my IMEI the first time I rooted the phone, but since they're helping me out, I'm just going to wait for the new phone and hope that that fixes it.

Take out your battery and look for the IMEI number that is listed underneath it. Write it down. Then boot your phone back up and go to settings, about phone, status, and make sure that the number your wrote down matches the IMEI number on the phone. If it doesn't, then you'll need to restore your IMEI and probably repair your ERI.

I had this exact same problem yesterday. I noticed it occurred when I left a 4G area. I couldn't connect to the network anymore and it prompted me to switch to global mode. My IMEI number on the back of my phone matched the number in the status too.
The way I fixed it was by doing an nvrestore in terminal emulator. You need to have a backup in place in order to do this. I believe you can find more about it in the IMEI thread in android development. last night i was flashing some old restores, trying to decide what rom to play with for the next week.
Phone booted from one of them and would not activate. Phone is stuck on Roaming ... i got the data warning, and also the triangle is near the signal area
I've tried flashing other roms, tried wiping all data/cache and starting over. cant activate phone

I didn't lose mobile data, I lost cell service. Cant make abphone call it says I'm not authorized (because it thinks I'm roaming)

Just did this and it rang, then said "Welcome to verizon, please hold while your call is forwarded to an external operator who can process your call using a credit card"
when i dial *228 i get a popup on the screen saying "a VZW Sim has activated your service and updated your roaming capabilities" but it doesnt place the call to activate or anything

yes definitely. This has something to do with the phone thinking im roaming. Up near the signal bars i have the little roaming triangle, and if i try to make a phone call it says call not authorized because i am roaming, etc
so this is definitely having to do with the fact that the phone is stuck in roaming mode
There have been two times i've had issues flashing and lost service for no reason. Turned out verizon suspended my account, call this number (800) 922-0204 and type in account stuff and your number. If it says your account is currently suspended it'll then ask if you want to activate it.. you give an emphatic 1!!! and for me this has worked twice.
Had some time so i ran to VZW. They replaced the SIM and tried hard reset like 4 times but nothing worked. They eventually offered me a free Warranty replacement by mail. I asked her to note my account but i didnt want the replacement just yet ... i said this because she mentioned id have to mail back my original, i was afraid they would see the root and charge me. Is that possible?

So I messed up big time and lost my IMEI and my ERI and never made a backup cause I always (stupidly) thought that if anything ever went south I could just odin a stock rom. Well I was wrong. I used odin to go from the cm10.2 nightly I was using to the stock verizon unrooted rom and I still had nothing. I have tried factory resetting and nothing. I stumbled across this guide and with It was able to restore my IMEI but not my ERI. After this I tried to reprovision the phone but that failed to fix my ERI problem. In my search I kept stummbling across topics that mentioned and linked to a thread with a download and instructions to use a samsung repair program that had a check box for fixing the ERI but all links to that topic and others like it say it was removed (something illegal/against XDA ToS?)
Does anyone know of another way to get my ERI back? I would just turn the use roaming on but I frequently travel to places where verizon is spotty and I would risk pulling real roaming data.
Any help or even a link to something else would be greatly appreciated.
Well, don't worry about incurring any additional charges -- while it says you are roaming, you are still connected to Verizon, and will not incur any roaming charges that you would not otherwise incur. I know this from personal experience, as my phone has had this same issue since December, and I have yet to have any issues on my bill. It is, however, annoying as hell, and I would love to be able to fix it, but as you said, it seems as though the only tools available to do this have been taken down due to a breach in the ToS (I am guessing that spoofing IMEI/ERI could allow people to commit fraud or other nefarious activities). It is a shame that tinkering is punished as it is, but I think that we might just be stuck dealing with this. If anyone has any other input, however, I would love to hear it.

If I was to go someplace without verizon service though wouldn't I automatically start using roaming data?
I just followed a guide that someone claimed to have fixed the problem by following this guide and then reprovsioning the phone after (*#22745927 to get to an 'enable/disable hidden menu' screen then *2767*3855# to reprovision)
If anyone can help in anyway at all it would be amazing. I have another year before I get to renew my contract and get a new phone.
