first get back the phone to working condition
to do that
try the troubleshoot point #4 from my soff guide ..link in my signature
then flash 4EXT recovery if you are not having it.
then go to recovery ..connect the phone to PC ..and mount usb storage in recovery ..you'll get a pop up in PC asking for formatting the sdcard ..do the format and its done
or you can try sd formatter ..its a pretty good tool in formatting sdcard
I panicked for a minute too.
Forgot to take out my 32GB SD card.
Wouldn't format in the SD slot of my laptop.
Did the above suggestion and all good!
Thanks
You can format SD card through 4extra recovery. Here's how I did mine. Pull battery put battery back in press volume down and power at same time, choose recovery, then in recovery choose format SD card, it will ask are you sure, choose yes. However if you format you must back up the information of on the card to your computer because format will wipe the card of all data. Once formatted you can restore your data you saved from the computer.hope this helps. Best wishes

I did a full wipe and i forgot to put in a CUSTOM ROM in the SD CARD..
what do I do!??! help
Don't quote me on this, but you should be able to put your SDcard in a card reader and put it on your card as PD98IMG.zip and Hboot will flash it.

Download the custom rom and place it on the sd card. Boot into recovery, do a full wipe, install custom rom.

This is true. I softbricked mine the other night pulling fonts around. Pull your sd card out and go to hboot. (you could leave it in if you DO NOT have a pd98img on your card) then reinstall after you get to recovery. As posted above you should still have your original rom on your sd card flash this if you don't have a cardreader. Good Luck!
where can I get this pd189.img
Download the ROM you want to your computer and rename it exactly to PD98IMG.zip and copy it to the root of your SDCARD. Hboot will automatically install it.

its easier to just put the rom on your sd card and boot into recovery..... Did you not make a nandroid back up? If you didn't then maybe that is something you might want to start making a habit of...
Get into recovery from hboot. There is an option in there to enable usb and you can use it like an flash drive like in android. Here are the steps.
1) boot into hboot (volume down then power button)
2) use the volume rocker to go to recovery
3) in recover scroll down to mounts and storage
4) scroll down to mount usb storage
This should allow you to view your sd card like u do in android when u select usb mode. From here you can put your rom on unmount and install. Good luck hope this helps.
I'm on CWM right now...I just got done completely wiping the phone and went to restore a backup I have and I get this error:
MD5 Mismatch!
And it won't do anything. I can't reboot the phone because I just wiped everything, so I have no where to go. Please help!

What does that mean?
I think my only option right now is to go to the store and buy a microsd card reader so I can put a rom zip file on my sd card and then get it up and running.

You're using CWM and you don't know what an RUU is...Great.
Go flash a stock ROM via USB and that way at least you will have a ROM back.
Or, go and use the mount SD card option in CWM to mount your SD card to the PC that way. Then you can copy over a ROM zip file and flash that way.
Alright I'm downloading a stock rooted rom right now.
Any ideas what caused this problem? Only thing I can think of is I made all the backups in cwm 4.0.1.5, but I went back to 4.0.1.4 because of the charging issues, and 4.0.1.4 was the one I was using to try and restore my backups.
I just want to know for next time.
I'm pretty sure that means there was an error signing the backup.
You also didn't have to buy an SD card reader, you can mount your SD card in CWM under mounts and storage

And what exactly does that mean?
After I get this up and running I'm going to try and do a backup with 4.0.1.4 and make sure it works with that version.
Whew, back up and running on stock rooted rom.

Do you have a physical sd card in the phone. The backups made in CWM are made to the 'external sd card' which is a micro-sd card you insert into your phone. If you are just using the internal memory I believe the backup will fail as it is looking for an actual sd card. Sounds like you are using ROM manager also. Probably the same reason it cannot download any other recoveries. But you should only flash a recovery with NVFlash. ROM manager does not install recovery to the actual recovery partition and does what people here call a 'fake flash' and is storing the recovery image elsewhere on the phone and tricking the phone to boot into it by having recovery apply an update.zip at boot. Use real recovery not fake RM recovery.
I figured it out, its just that my SD card went bad it wont even let me format it again. I pit a new SD in there and its working just right. Any one have any tips on the SD card card dying?

Is it a cheap Chinese SD card? If so just break it in half and throw it away. If a good card try deleting the partition, create a new partition, make it active and try to format it. There are also freeware apps you can download to try and repair sd cards. if that fails toss it, its life is over. :-( Not worth risking your device on a bad card. I had a cheap Chinese chard in a BlackBerry and the BlackBerry kept running erratically and rebotting, etc. It turned out to be the sd card causing all the problems. Once I put a new one in the BB worked with no problems again.
Hello, i recently returned to stock. Afterwards I re-installed CM9, now my SD card won't mount in anything, when i try to mount it in CM9 it just immediatly says SD card safe to remove. any help/suggestions would be appreciated.
Also I have tried the method in this post http://forum.xda-developers.com/showthread.php?t=1630459 but it did not help.
Do u have cwm or twrp?

I am currently running cwm

I recently had a problem with the sdcard not being mountable too. Mine was corrupted so I had to reformat it.
In was using twrp recovery but it is probably the same with Cwm. Try to mount the sdcard from recovery with your pc. The pc might say it needs to be reformatted before use. If so, you just format it to fat32.
Hope it is this simple for you too.
No, the card will not mount in recovery.
Would returning my phone to stock help me in this matter at all?
Cwm can't mount sd card usr twrp

I tried twrp and it still won't mount.
Did you look at this thread? http://forum.xda-developers.com/showthread.php?t=1630459
It is a common problem.
OK i ran through the process in the post again, for some reason it seems to have worked ths time around. Thanks for the help!
Hi,
I just tried to update the radio on my phone, and now it is stuck on boot. I am shown a white screen with the htc logo.
I have tried to go into the recovery menu, but as the radio is located on the SD card it automatically tries to reinstall, and doesn't allow me to reformat or otherwise. How am I able to get out of this?
I don't have any access to the SD card otherwise to delete the file!
adb reboot recovery
I hope you enabled USB debugging...
If not, you need a SD card reader or a second phone to delete the zip from the SD-card

I don't seem to have adb access when I turn it on. I did however remove the SD card from the slot, and perform a factory reset / clear. Now boots up, however the radio doesn't work.
So people are aware, if you're using Reaper, don't update the radio to the ICS version! Otherwise I assume you will be in the same pickle I am in.
