Today I had about 2 dozen sdcard folders that could not be read by Titanium, MyBackUp Pro, Linda Manager etc. Did nothing since a TWRP backup last nite that went fine.
Tried to wipe card to copy a bu from pc but got notification in windows of corrupted files and they would not delete. Those included the above mentioned folders. Also, no luck with trying to use TWRP to wipe card.
Didn't try Fix Permissions with TWRP as didn't think that would apply. So I unmounted card from pc, still pluged in, and selected Format SD Card icon in TWRP.
It stopped on Verifying partition sizes after it started the Formatting /sdcard sequence and has been there for over 20 minutes. So, much easier with a removable card. What would you suggest I do next. Can't take that long to format with TWRP, or can it--?
I normally use SD Formatter and it did take awhile. But, when using the recovery like AmonRa etc only a couple minutes. Obviously not a full format.

Well, I had to back out of formatting and format with sdformatter. Then sdcard wouldn't mount so I did the clear storage method and got data back on sdcard.
Problem is now after reflashing twrp and cwm the bootloader screen still shows the "clear storage" choice. So, even though I can boot into recovery, I could not see any bu's in twrp to restore. I did in cwm and restored Zydroid 3.3.5.
I will see how it runs, but why is clear storage still an option in bootloader, I thought that disappeared with a custom recovery--maybe not??
And why no bu's showed in twrp to restore, but did in cwm-- The only thing I did different was flash twrp thru fastboot as opposed to thru Goo Mgr originally.
Two final notes then I will retry twrp soon and move on:
I don't pay a lot of attn when backing up or restoring. I did watch this time and saw cwm back up recovery as a default. Twrp has that as an option I never selected--not sure it matters--any thoughts--still trying to figure out why twrp showed no bu's when I selected restore, not even the backup folder. But, the folder showed in File Mgr.
And do you guys with custom recoveries show a Clear Storage option as is available with a stock recovery. I don't remember seeing one. If so, why flash a stock recovery to Clear Storage and get sdcard remounted--
If the Clear Storage shouldn't be there, I still may have recovery issues. Although, all good atm on cwm--
TWRP and CWM has different backups iirc, they can not flash backups made in the other.

problem was i reflashed twrp after restoring my sdcard
when i went to restore my twrp bu thru twrp the twrp bu folder never showed, just a blank screen after tapping restore
thing is i never tried a restore with twrp, so don't know if it ever worked--
solved one issue thru experimenting
you can't change bu name that shows in file mgr. it didn't have a date and I gave it one. then it doesn't show in recovery when you select restore. I did that to give it a date. but, found out when viewed in recovery after tapping restore the date is there not the HT24HW403671 name.
saw on some guides with cwm that clear storage was mentioned, so guess it was always there and didn't notice as never used it
back on twrp and will see how it goes
one question--if you can't change bu name, how do you differentiate btw diff roms in recovery bu folder--keep only one and put rest on pc?
All Resolved--NP with the Folder Names, didn't know the dated bu's were inside the HT folder--
Hopefully no more sdcard unmounting on its own--

I haved tried two differnet roms Trickdroid/Axiom (have not gone back to LeeDroid yet). Have run twrp and cwm. Deleted SD Card Speed Boost. Have tried mounting thru phone and twrp.
I still get missing/corrupted sdcard folders from DCIM to Downloads and alerts of folder doesn't exist and need to reformat card. I have reformatted with sdformatter and thru phone. Will lose personally made ringtones.
I have gone back to Axioms v01s kernel from the oc vo6. I had that running at 1890 and 1809 and 1674--no help
Just fastbooted the v01 so will see

did a good backup, all ok--no lost folders
when I open two windows panes on pc to copy bu to pc from phone and backup sdcard to phone issues start
the right pane which is the phone storage pane starts to lose the Removable Disk H disk icon, which is phone. Pane flashes and goes back and forth btw showing card contents and not showing H disk. Also showed empty H folder as nothing on card. I unmounted and remounted phone to pc and all was there. Unmounted and checked card contents with phone not connected to pc--all ok
Beginning to wonder if it is usb port problem--something is losing connection and/or unmounting remounting on its own. Maybe why some folders got corrupted before--still confused--
get popups on pc occasionally that state devise can perform better on a differenet port and sometimes device icon does not show when phone plugged in. I have changed ports, on my third one that only was used for mouse, will see. I have once or twice reinstalled htc drivers--

I have Open Recovery and was getting ready to flash Kahl's Galaxy and when I went into my nandroid, my back ups were there and the Galaxy folder, however there were no options to "tick" (system, data, cache,...). It was all empty. I can see the nandroids on my PC when I plug it in but can't flash them. Says something like, "Nothing to Restore".
Tried reformatting SD card
Re-installing Open Recovery.
SBF back to stock
32gb card (fat, then 1g ext3, then 125swap- and all are ordered correctly)
Any ideas?

They are set up the correct way. Even my old bakc ups aren't showing correctly either.
Another clue to the puzzle:
When I go into my Gallery, I get a triangle at the top that says "failed to read/write sdcard. Gallery may not work as expected"
Maybe a mounting issue. Everything was working 2 days ago.

but i got also those problem like yours when my sdcard corrupted and can't be mount on phone, try reformating your mem card.
At first I thought it was the card (and still kind of do now) but it seems that Root Explorer, even though you toggle it to r/w, sometimes it kicks back to r/o and that is what is screwing everything up. When reflashed my 2.1 sbf and started from scratch, re-installing Open Recovery, the sdcard was ticked at r/o, so Open Recovery, as hard as it tried, couldn't read the files.
This also created the problem with the media gallery b/c when I tried to save an MMS to the sd card, it would show up as a broken link/pix. This is actually what led me to the r/o vs r/w Root Explorer problem. Ticked it back to r/w and tried to re-save the MMS pix to my sdcard and viola, it worked. Open Recovery shows all my Nadroid back ups and the sub-folders within.
Now if there was some way to figure out how to make Root Explorer r/w all the time and avoid the constant fear that after I reboot sometime, it hasn't gone back to r/o and messes with my head.
I'll tell you that this phone, as frustrating as it may be, is always a learning experience.
1. the software that running on your phone now is came from a "fresh" file master(from market or computer) or from the backup from your sdcard after it crash?
2. is the fat partition set as active?
3. when reformating the card you use card reader or your phone(use phone as card reader) ?

my problem at that time is quite the same as yours.
and it all happpened bcause i ran the software from the backup file( not a fresh one), i format the card using card reader(maybe my card reader is not run well, so the formating process not run perfectly) and i forget to set partition as active lol
when i tried to use a fresh file master, format through my phone(use phone as card reader) all the problem is gone

I've the same problem in the gallery. (failed to read/write sd card)
I can shot some pictures an then the cam freezes. After that the whole xt720 freezes.

I tryed 3 different sd cards - all the same problem.
- the orginal class 2 card
- a 32GB class4 card
- a 16 GB class10 card
I tryed also differt roms:

Is it possible that the SD Card reader inside of the phone is damged?!
I need help. i'm going crazy...
USB reader in phone works fine for me. It isn't the card per se, but rather something messing with it to make it glitchy. I have a 32gb class 10 card that is A-OK but I also tested a 16gb class 6 card I had too and it did the exact same thing on me.
I have narrowed it down to the Camera function. For some reason when I take pix it won't write to the card. Was just on a short vacation with my wife and kids and took a picture of my boys sitting on a canon and lo and behold, it didn't write to the card. I think what I did was take the card out, reboot w/o it in and then once phone was fully powered up, inserted my card and remounted it. Picture wasn't there but could open my media gallery with no issues.
To tell you the truth, I have tried so many ways to figure this out, that they are all jumbled now. Once it happens again, I will pay more attention to how I fix it and post it. Really, I think it all comes down to an issue with the camera and the ROM not mingling together.
Running Steelblue ROM, but had same issues with Personared too.

I'm sure you've done this, but for completeness, some other things to check are to make sure all partitions are primary and that the fat partition is marked "active" (or "boot" depending on which partitioner you're using)
Been awhile since this was an active thread but I did say I would post a solution when I found one. Seems that something is corrupting the sdcard when you take a picture. Not sure what is causing it but here is the fix.
You will lose that picture, but go into your media gallery and once it all populates in, a triangle will scroll in your notification bar that says sdcard failed to read/write. This seems to screw with all kinds of things.
Solution: delete the picture that doesn't have a thumbnail, pull down notification bar and hit the error message. Unmount sdcard and force stop your media gallery. Remount the sdcard and close out your settings. Reopen your media gallery and you should be good to good. Simple as that. No reboot, battery pull or reformatting needed.
Seems that camera corrupts something during during read/write and it trickles through all SD files, including OR folders.
Im having real problems with my Nexus S on 2.3.4 rooted
When I start it up I just get constant force close messages from a load of apps, including system apps. I cant use the phone at all because everything crashes. I've not really made any changes apart from manually flashing the 2.3.4 update and thats been running stable for about a week
I figured I would try and flash a new ROM and/or go back to stock 2.3.1 ROM which I found in the forums here to fix it but whatever I save to my SD Card does not appear in clockwork recovery when I select "Install from zip"
I then tried to copy the ROM again which was showing on the phone through windows explorer, then when I unplugged the cable and plugged it back in, the file had disappeared and any files/folders I created on the SD Card have gone.
Any help would be much appreciated

All of the apps crashing seem to complain about the database. Im getting SQLiteExceptions from everything, either unable to find file or cannot write to a read-only database
I've tried to lock/unlock the boot loader, wipe everything via recovery (clockwork and stock) and factory resets but everytime i restart, its always the same as if i haven't wiped and the errors keep on coming

i think these problems are all linked to being unable to save to the sdcard/internal storage. Is there a way to flash a rom without using the internal storage? say have it linked to the PC and do it from there?
Ever since I first got this phone, I have been unable to detect/mount the SD card in CWM, and under "Storage" in "Settings."
Oddly, I have no issues accessing "sdcard" in the My Files folder, or in Root Explorer.
Whenever I flash a ROM, I have to select "choose zip from internal" instead of "choose zip from sd card."
I have no issues with doing this, however the reason I think that I should fix this is because I keep having problems with Market FC's and "phone is not compatible" error messages when trying to get apps from the market. Also, I am unable to use Nandroid in CWM because of this.
I have followed the instructions here except I obviously was unable to format the SD card.

So no more issues with the market anymore. I'm still interested seeing if there's anything I can do about my sd card not mounting.
I've had the same issue, ROM Manager tells me that the SD card needs to be mounted when it already shows as mounted in settings OR it says "unavailable" in the space for both internal and external.
I can fix it by removing the SD card, turning off the phone, inserting the SD card, and turning the phone back on.
It almost always reoccurs every time I flash a new ROM.
I'm having the same problem, and I've tried to use seanscreamz fix in the link you posted but that didn't work (but, like you said, since I can't mount the sd card I can't format it ).
Does anyone have another solution? Even after going back to stock, wiping data, cache, system etc. nothing seems to work. I can't use ROM Manager, can't restore nandroid backups, can't mount the damn sd card!
I've attached a picture of the Storage display if it helps (USB Storage appears to be my sd card and the sd card is 0 out of 0).
Woo hooo!!! Fixed it. I can now run Nandroid backups and presumably restore them.
I'm not sure what worked, but I think it was flashing the latest CWM from ROM Manager. The Odin-flashable cwm I had didn't have the options for "Backup to Internal SD Card", "Advanced Restore", etc.
Other things that may have helped:
--Tried seanzscreams repair here: http://forum.xda-developers.com/showpost.php?p=21424179&postcount=1 (but obviously couldn't format the SD Card that I couldn't mount).
--Flashed back to stock, wiped data/cache in 3e recovery.
--Rooted, then clean install of Alien 5.5 rom using DARKSIDE SUPER WIPE first (new! found here: http://forum.xda-developers.com/showthread.php?t=1487706)
But I'm pretty sure that flashing the latest CWM from ROM Manager was what fixed it.
I followed the folder tree properly, i think. its on my external under clockworkmod/backup/<folder with date>
I did find the option to write new backups onto the external, but that has "mnt" in the folder tree... its under "mnt/sdcard/external_sd/clockworkmod/backup"
So i think i did it right??
I was hoping to keep the backups only on the external, freeing up some of the internal space. I did delete the oldest one off of the phones internal memory and went into recovery and it could only see the newest one - the one still on the internal. *but* i did check this before finding and changing the setting for the program to write new backups to external. would that make a difference??
Also, using rom manager and saying reboot into recovery used to work. But i had something weird happen where my phone randomly said it was not installed anymore. so i redownloaded it, and since then, when i click on reboot into recovery, it does nothing. i push ok and it will just freeze on that screen, giving me the ability to press home and it returns to the main screen as though no command has been made.
help??

I recently upgraded my Blaze to CyanogenMod (v11 nightly), now my apps (Camera, Swiftkey, etc.) do not recognize my SD card. I was running stock before this.
In File Manager:
- I can view the external sd card at \storage\sdcard1.
- It appears there is an internal sd card as well at \storage\sdcard0
- There is a folder called \sdcard which appears to link to \storage\sdcard0
- There is a folder called \external_sd which links to \storage\sdcard1
- sdcard0 is empty
I believe this to be a mounting issue, but if it is I cannot figure out the correct mount command in terminal emulator. Thanks for your help in advance.
I know this is nearly a year later, but I ran into this exact same problem while trying to flash an old phone. I was fortunately able to figure out to fix it, and I'm posting this for the sake of anyone else who might have trouble with it as well.
I initially noticed the problem while attempting to download and open files, which kept giving me an error that the files couldn't be located. Using a file manager, I noticed the directories that the OP described above. I believe the internal storage for the Blaze is 4GB, which is partitioned into two parts: one for app data, and the other for user downloads, pictures, etc. I realized that I only saw one of the partitions being displayed on the storage settings screen. Connecting via USB to my computer didn't show any internal storage; I couldn't access any files from the phone besides the ones on my external microSD card.
I booted into my previously-installed ClockworkMod v6.0.4.8 (by holding the volume keys + power, letting go of power after the Samsung splash screen) and went into the 'mounts and storage 'menu, scrolled down, and clicked on the 'mount USB storage' option while connected to my computer. Surprisingly, the internal storage was recognized (specifically the partition containing user data) and I was able to pull all of my files from there that I thought I previously wiped when I went to flash Cyanogenmod 10.2. Somehow, those files were still retained. Now, I'm not exactly sure what happened, but I think all this previous data, which didn't get wiped, prevented the new ROM from mounting this partition (sdcard0), and perhaps led to a path/directory issue when I went to download and open things from it. For some reason, as the OP noted, the sdcard0 storage showed up as empty when I used a file manager app to look inside.
I deleted the files from this mounted storage via my computer, then went back to the 'mounts and storage menu' and formatted the /storage/sdcard0 storage with the 'default' setting to wipe that partition. Then, I wiped user data, cache, and dalvik cache, and let my phone reboot. I did not reflash Cyanogenmod 10.2 after, and I was able to boot into my phone and setup my phone. (I'm not sure if you have to reflash your ROM, but I was able to boot up to a fresh setup without it). I am now able to see both internal storage partitions displayed on my storage setting, use a file manager to see the files in sdcard0, and download and open files.
