You need to get the CWM tar file from one of the other rooting/CWM install threads (search). Then make sure only auto-reboot is checked (NOT re-partition) and click PDA. Select the tar and click start.

thanks, even with the error it asked for superuser request when i booted everything is working properly

From the sounds of it, it sounds like this is a guide for stock, unrooted.
ODIN-> CWM-> flash zip.
But if we're already rooted, do we just need to flash the zip?

Step One: Download SuperUser zip & copy to your root of SDcard
Step Two: Download and install HTC Drivers
Step Three: Download Revolutionary: S-OFF & Recovery Tool (Note the download link is at the bottom of the page. You will need to enter your EVO’s Serial number which is located under the battery. Make sure you to copy/write down the beta key. You will need it later)
Step Four: Enable USB debugging mode & fastboot on phone (To enable USB debugging go to Setting – Applications – Development – check box for USB debugging) (To enable fastboot go to Setting – Power – check box for Fast Boot)
Step Five: Connect you phone to you PC - Select USB Charging
Step Six: Unzip Revolutionary tool and run it as admin (to run as admin, right click and select run as admin) (make sure you install recovery image when asked) (Note after the tool is complete you may be left in fastboot mode, this may happen if recovery doesn't flash or something goes wrong with flashing the recovery. If this happens just select bootloader then select recovery to see if you are able to boot into recovery. If you are not able to boot into recovery or it hangs try flashing AmonRA Recovery from bootloader.)
Step Seven: Boot into recovery
Step Eight: Flash SuperUser and reboot your device.
Congrats you are now rooted with S-OFF!!
FAQS
Q: When I run Revolutionary tool it just sits at "waiting for device".
A: Make sure you have HTC SYNC Closed and or Removed in order for this to work - Thanks @smallba
Q: I'm stuck at "zerging root" nothing seems to work.
A: Again make sure you un-install HTC sync and turn off your antivirus and/or firewall
Q: I keep getting invalid key. What am i doing wrong?
A: The beta key is case sensitive so make sure you get it right, try to copying the key into notepad to verify the characters. The Revolutionary tool will give you your device serial number, verify you entered the correct SN. If you are still having issues try regenerating the beta key. If all fails try re-downloading the tool.
Q: I'm having issues flashing anything from recovery.
A: If you are having issues flashing/loading new roms try updating ClockworkMod Recovery from market via rom manger or flash AmonRA that seems to fix it
Q: How do I flash AmonRa Recovery in bootloader mode.
A: Download AmonRa Recovery PC36IMG.zip to the root of your sd card. Boot the phone into the bootloader by powering the phone down, then press the power and the volume down buttons at the same time.Once the phone boots into the bootloader the phone will automatically recognize the zip file. It will ask if you would like to update, select yes and let it do it's thing and you will be good to go.

Nice walkthough. However, I think they shut it down temporarily because the beta key generated for me isn't working... I know I'm entering it right I got the same key everytime. Hopefully it'll be back up soon.

Try doing a copy and paste
To paste in CMD you just need to right click.

If you didn't download clockwork. Get the pcimg.zip of your favorite recovery and put it in bootloader

The tool will ask you if you want to download a recovery image. just make sure you have internet connection and hit "y & enter"
It asked me if I wanted to flash Clockwork, I said yes. Then it said downloading then success. Then it left my phone in the bootloader without a recovery option.

Probably a miss type , if u have o or 0 or I i or l (L) , the font can confuse u , it happened to me

Make sure you have the HTCDrivers installed (you may have to reboot)
Also make sure to check usb debugging
I had to run revolutionary after openeing CMD first, the running revolutionary.exe.... After that it allowed me to paste the beta key inside it. I'm now rooted again!

I have tried to root my sensation device, I used a guide to do that(forum didn't allow me to link cause I'm a new user), and everything looks fine, my phone says s-off and everything looks alright, however I tried to use titanuim backup, said it requires busybox, installed busybox, says "the su binary was not found on your phone", installed superuser, and tried to update the su binary, and it fails at "checking current install path" and I hit a wall. where have I gone wrong?
also I accidentally hit the SIM lock option on bootloader screen, I don't know what that does, so is this bad? and if so how I fix it.
If you have guessed that I'm a noob, you're right, I just got my first android phone a month ago.

Yes that page . An older version of recovery is attched on that page. It can be downloaded here.
Now,
1. Copy the PG58IMG file into the Root folder of your SD card (The main Folder of your Memory Card)
2. Turn off your phone FUlly and reboot into the Bootloader/HBOOT screen
3. Press Volume up to Accept when it asks" Do you want to start the update"
4. Press Power Off
5. Delete PG58IMG from your memory card
6. Reboot
Alternatively ,
If you have fastboot and adb setup on your pc then download the recovery from here.
Then flash it by the following command in fastboot
"fastboot flash recovery your-recovery-file.img"
After flashing recovery download the flashable su binary from here.
Put it the root of your sdcard.
Flash it via recovery.
Now you are root.
Note: **** If you get installation aborted while flashing su binary , do toggle signature 3 or 4 times . Then try again ****

Need more info. What ROM.....S-on, S-Off......what recovery......you know. As much of a description of what you're working with as you can think of. How are you flashing, what had you done right before this happened if anything, etc., etc......

i was trying to install rubix senceless 4.0 , cyanogen mod 9 , bulletproof 1.1 and i have cwm recovery 5.5.0.4 and im just flashing all thru cwm Recovery it will install okay then once i reboot bulletproof rom will hang on the t-mobile mytouch 4g slide other roms like rubix and cyanogen will hang at the boot animation screen prob i did something wrong cause when i wanted to install a new fresh rom i thought formatting everything wont damage the phone but here is what i format
on cwm-based recovery5.5.0.4
wipe data/factory reset - passed but with an error E:format_volume: make_extf4fs failed on /dev/block/mmcblk1p2
wipe cache partition - passed
advanced/wipe dalvik cache wiped - passed
wipe battery stats - passed fix permissions - passed
This is why I have cwm v4.0.0.9

Have you by any chance tried out CWM 6.0 and decided to go back to 5.5.0.4? Did you extract the "boot.img" file from the ROM & fastboot flash boot boot.img thru ADB AFTER you flashed your ROM.zip file since you are S-On?

Hasoon's toolkit is for unlocking, installing CWM & superuser if my memory serves me though. You can flash your ROMs thu recovery (CWM) but with S-On you will have to extract the "boot.img" file and flash it seperately thru ADB because HBoot will stop an unsigned file it sees as a Radio from flashing if S-On.
There ARE a couple of other versions of CWM....5.5.0.4 & 6.0.0.0. (6.0 has some REAL issues though and has bricked several phones so IF you notice it....don't USE it.)

If you flashed your ROM and then the boot image after you did that it shouldn't be a problem. Did you read the post above yours and answer those question though? Did you check the link I provided and understand fully what it was talking about? That is VERY important.

i extracted just the boot.img from the rom put it in a new folder then copied the rom.zip file to the same new folder im not doing the command prompt right? please help sorry im a noob

if you dont you need to do this ASAP because when you are on you have to fastboot .img files to make it work im not too familiar as i am s-off. follow my first link to set up adb and fastboot then go to the link weekend posted. (which is stickied in the development section).
Move your ROM.zip file to the root directory of your phone first. Then put the boot.img file in the root directory of your computer since it's already in the windows path. that's " C:\ " Since you already have Hasoon's toolkit installed we'll use that instead of putting you thru the ADB installation right now for time & simplicity's sake.
Go to Windows Explorer & navigate to the folder you have his toolkit in and copy the name of the folder down EXACTLY. Then you can pull up your command prompt window again. When it initiates it will say C:\ sumpthin....sumpthin...sumpthin. From there type CD\ and it will "change directory" back to C:\ THEN you can type cd\ "whatever you named the folder that has Hasoon's toolkit in it".
That will take you to that folder just like explorer will do. Just let it sit there for a second.
On your phone in Recovery you wipe user data/ factory reset 3 times then you can....
Wipe cache 3 times and toggle down to "Advanced"
That will take you to another screen where you will see "Dalvik Cache"
Wipe that 3 times as well.
Then hit "Go back" & return to the previous screen & select "install zip from SD card"
Once complete leave your phone sitting & still plugged in to the usb
NOW....go back to your computer and type....
reboot bootloader
fastboot flash boot boot.img
fastboot reboot
Once your phone reboots, leave it alone for 5 minutes to let everything settle in and you should be "good to go".

You're right. It IS very easy. Either way should work but sometimes my mind doesn't go down the easiest path.

I am not responsible for any damage you may cause to your phone with this steps, this is how I did it and work flawlessly for me.
here is a summary of steps to update to FP5.from any rom or kernel you may be:
1.download EP4 Odin Package here: http://www.imnuts.org/downloads/charge/odin/CI510_VZW_EP4_ALL_ONE-LTE-CP_REV03_user_CL706183_ship.tar.md5
2. download Pit File here: http://www.imnuts.org/downloads/charge/odin/charge.pit
3. put your phone in download mode, and Flash the Pit file and the Odin Package together, it should pass
4. when it finishes, reboot your phone, you should be on Stock EP4, As soon as you connect to the internet, you will see the system update popup, accept it and reboot to install it,
5. once you finish installing the update, this will put your phone in FP1, again, as soon as you connect your phone to the internet, it will prompt you to install the update which is FP5, accept it and install it
ROOTING:
You can follow the rooting process that I used in this post, it will help you avoid the problems I had:

You need to remove the "]" on the pit link because it breaks the download link. Took me a couple minutes to figure out why it was giving me a blank page.

Using Odin3 v1.82 I am getting a "MD5 hash value is invalid" from the EP4 file. I tried re-downloading it and got the same response. Tried flashing only the EP4 file without the PIT file and no change. Tried changing USB ports and it didn't do anything different.
Any ideas, anyone?
Have to keep re downloading until you get it right.

MD5 Hash value invalid means a bad download, or the uploaded file was corrupted, since we downloaded that file, is proven to be ok, so just download it again, or use a different computer or browser to download it
So yeah I'm asking how to do it..

Totaly agree.... someone should make a more noob proof topic, with links to the required stuff you need. The topic that exists is comparable to this ( no offense author ) : " How to draw a horse? Step 1) you take a piece of paper and a pen.... Step 2) You draw the ****ing horse.
I used that guide and I used method 2 the fast boot way, just take it step by step, start by downloading the fastboot link then get the update from Asus and extract it twice and move the blob file to the fastboot folder the just follow the steps mate

ive had problems with cwm in the past so i would suggest using twrp in that thread

I agree with Rob. Come to think of it, I haven't read one time where someone was using CWM on the new bootloader. I'm almost positive that it isn't even compatible.

It says in the thread to use TWRP and then it walks you through the process.
First using method 3
"Method 3: Update using TWRP recovery
Notes:
You must have TWRP installed
You need to put the update file on an SD card
This was the method I used since I wanted to prevent any bugs due to old data and to ensure the new partitions from 4.2 would be OK
You will keep your data as long as you don't wipe Data and Factory reset (including internal memory)...... etc"
And then below that :
-- Installing TWRP (Recovery) --
Since this new update brings a new bootloader, older recovery versions from 4.1 won't work at all. Don't try to flash one!
This guide will give you a patched recovery that works (almost) fine on the new bootloader. And yes, it can read/write to an external SD just fine. Follow this steps to install the new recovery on your device:
Connect the tablet to the computer
Reboot the tablet using the proper button on the power menu, holding Volume down until you see three icons (RCK, Android and Wipe Data)
Download this file and unpack it (it contains fastboot and a patched version of TWRP 2.4.4.0)
Open a command line on the extracted folder (you can use the instructions I wrote in Method 2 if you don't know how to do this) and type
Code:
fastboot -i 0x0b05 flash recovery twrp.blob
Wait for the script to complete. Once it is done, reboot the tablet by holding volume down and power button for 10 seconds or using

fastboot reboot
on the command line. Don't enter recovery directly!
You have now the new recovery for Android 4.2 .
So if you want to flash TWRP follow the instructions and then use method 3 to update and then reflash TWRP again

The link I found does not allow me to download the file from asus website.. it doesn't give me a drop down for OS before I see any download for the Stockrom..

Yeah just go back and reselect support, downloads and type in your device and do it again that way.
