Turn it off completely and then plug it in, don't hit the power button and watch the green battery, if it isn't going up then either the battery is dead or the cable doesn't work or the port is broken or the charger is broken or you have a dead wall socket.
I'm pretty sure you know how to isolate each of those issues.

Yep, if the battery looks like it's charging but it isn't, probably means that it's a goner.

Most likely your battery is shot, if you're under warranty samsung will send you a new one if you claim it.
If you by some freak chance know where to find a cadex machine you can test the battery for real.
but most likely your battery is shot :|
Heres a link to chat with a rep and see about sending your battery in for warranty replacement, beats spending 50-60 bucks.

Hi all, I'm not talking about the recovery triangle with the exclamation point, but when charging the S in my car, I noticed that the battery icon in the notification tray would charge for a few minutes, then stop charging and the battery icon would just go static at 75%.
So I turned off the phone, and was then presented with a black background and white battery icon, with a flashing exclamation point.
I unplugged, and plugged back in, and the battery started indicating it was charging via ascending white stripes, But then it stopped and changed to an exclamation point again.
The battery was only 50-75% full.
Could it have been overheating in my windscreen (I'm in LA)? Or is there some other issue with the charger. What does the ! mean?
Is your battery original? Maybe your charger is not supported. Not sure about this. Can you take a screenshot?

I think that your car charger doesn't deliver enough power to charge the Nexus S battery. Some smartphones are draining more power then they are becoming from the charge unit when connected to the car.

This does not explain the error when charging with phone off.
I would exchange the phone under warranty if possible.

It's never a good idea to leave a phone, or any electronics, in a hot car during the summer. The combination of a hot car and heating up the battery while charging could be bringing the phone's temperature too high. It could be the charger isn't wired up for the standard micro usb charging. I've seen this with lg chargers and phones and data cables. It could be a bad flash.
If it can fully charge inside on an ac charger, then I might suspect the charger in the car. If it acts the same with a different car charger then it could be the phone. Try a 12vdc usb female charger with a data cable plugged into the phone. Try flashing the phone back completely to stock and do more process of elimination.
Thanks.
This is a brand new phone with original battery, stock ROM, not even rooted.
The charger worked fine always on its previous owner, a Nexus One. It is hard wired into my car so not easy to change.
I wonder if the Nexus S has overhead protection that stops charging?
A screenshot is quite a challenge ;-)
Edit: Here's the answer!: http://www.google.com/support/forum/p/Google Mobile/thread?tid=09793bd3fcb30e61&hl=en
43 degrees Celsius is the trigger point where it starts/stops taking a charge because of heat.
Heat
Its a samsung battery protection system the same system that wont let u over charge the battery once the battery gets too hot it will go in to this protected mode wit the exclamation mark
had it on both my samsung vibrant and the nexus s 4g

charge your phone to a max of 2 nd half hour and if possible switch off and charge (it will charge your phone in less thn 2hrs)
dont keep on charging it after every small usage....

Maybe your charger cable is broken so its not charging or maybe the connector in phone has damaged...
Just check by charging other phone with that charger!!!
And if its charging than your connector is gone and if not than your charger is gone
You have to go to service center!!
And plz check by once changing the rom!

Hi Guys,
I have a nexus 5 and I noticed that my LG WCP-300 continue to charge even when the battery is full.
The charger works fine with my Nexus 4, it stops charging when the battery reaches 100% capacity but not with the N5.
I'm wondering will it affect the battery if I keep it on even tho the device says "Charged" ?
Thanks in advance
Interesting. My Nexus 5 just arrived and I'll be curious to see how my LG WCP-700 charger handles charging the Nexus 5 when i get off work.

I think they finally fixed the wireless charging. My N4 would stop then drain down then start charging again. I never knew if it would be fully charged when I took it off the charger in the morning. I'm certain the battery will not overcharge.
Off topic, what is the use of wireless charging other than not using a wire to charge ?

Apart from the convenience of just putting down your phone to charge it will save your USB port from damage so if you need to fastboot for repair / warranty / general flashing you know it will be in good condition if the time comes. My galaxy nexus USB port broke, plugged in the charger and noticed it wasn't charging after a while and plugged the plug to see smoke come out the port and the phone was very hot! It may have caught fire if left. So now that phone has a dead battery, luckily its removable so could be charged separately with out the phone. The screen would turn on and drain battery too. I'd fully recommend getting one if you can, I intend on getting one in the future.

I aslo have the WCP-300 with my Nexus 5. I am experiencing the same thing.
I wonder why they changed the charging. I know my Energizer Qi charger also does this because the charging light always stays lit blue.
Wireless charging on Energizer
I have the energizer charging pad and noticed the same. My Nexus 4 would stop charging and if it fell back to 98% would charge again. The Nexus 5 just stays charging at 100% and I have read that it can damage the battery overcharging like that. Can someone confirm that for sure?
nothing on this?
Bump
Not unique to the charger

Just for the sake of clarification. The same thing occurs with the Nexus 5 using the Panasonic QE-TM101 which (as above) worked flawlessly with the Nexus 4. So it appears to be the phone (not the charger). And in spite of people assuring us that this won't damage the phone (battery), I can't help but think that the heat can't be good for the battery and if it wasn't "actually" charging after it hit that 100%, then what's generating the heat (it has to be actively charging to create that heat). I plan to do some controlled tests to see if the same problem occurs with my OWL (Qi compatible but not Qi certified) chargers, and see if the charger is cycling on and off during the charge period or staying in charge mode. But every time I've awoke in the morning the Panasonic charger has actually been charging (charge light on) with the handset very warm.
I'm afraid that if it's not cycling then over the long haul the battery (or possibly the phone) may suffer ill effects due to the consistent heat.
Hi, i know this subject is already exists in here, but it still have no answer. My sensation need 6 - 8 hours to fully charge from 10% to 100%, is there someone can help me to solved my problem?
This happen everyday, so don't ask me to remove the battery and try again because i've tried it.
Are you charging your mobile via a charger connected to a power outlet or are you charging via usb connected to a pc? How old is your battery?
it is a common HTC's issue after rooting the device
everyone has percentage mireadings while charging the device

Has it always been like that or started happening? When mine started doing that I replaced my battery, charging times went back to normal and battery life was a lot better also.
It happens to me with my original battery which I think is dead because it lasts not much time. Buy on my anker battery it charges really fast, with the same charger. So the problem is your battery I think, as on mine

What rom are you using. Sometimes my sense 5 rom misreads ac for usb and charges damn slowly.
Happy to help.
If your charging via usb connected to pc then charging will be half day, its because usb port is for data sending and not charging, so USB outputs very low volt charge and so device like phone charges so slow
If your charging with adapter into AC outlet, you could check if your using right cable(there are data cables where there are little wires inside so again it cannot output that power to the device and there is cables for data+charge with bigger cables inside)
P.S. Stock HTC cable provided is not the best wire....
Mine sensation is charging about 2h40min from discharge(10% - 6%) to full charge(100%)

It's already happened for along time.
When first purchase it is not like that.
I've replace the battery, but not with original one nor anker, and the charging time become faster about 1 hour than before

I just bought a wireless charger, and it works great...except it seems to stop at 100%. Rather than keeping it topped off, it will start draining after it completes it's charge (i.e. if i wirelessly charge overnight, it will have 85% battery in the morning)
Thoughts? Is this the way the MAXX works, or is my charger defective?
I believe, and dont quote me but, thats the smart battery in the phone that keeps the phone from overcharging.

If you lose 15% of battery overnight while the phone just sits there, you might have bigger problems than just the charger. But to answer your question, I have the oval shaped Nokia charger pad from AT&T. It will get the battery to 100% and maintain it. If I pick my phone up off the charger after 2 days, it will still be at 100%

The phone when charged to 100%, it will stop charging until the phone reaches arnd 97% then charge back up. And it repeats until u unplug it. It prevents over charging. Also, he is right. If it is at 85%, its either ur charger cannot charge as fast. Or ur phone.
