I updated mine to ICS using OTG, and did a full wipe in the process. It now works just fine if connected to PC, and just fine when not connected.
Try to clear cache (won't hurt anything). If that doesn't work, try a factory reset (backup up important data first)

Yep, mine just can work if it connected to PC and run fastboot in order to open it, if not, it always stuck on the boot loops and it restart again and again.

Recovery console

Hey guys, here is the deal. I did toasts instructions but forgot to wipe the cache/data so it was just looping along the 4g screen. So I reboot using volume down key and reflash the original PC36IMG.zip, then I do "adb reboot recover" and restore my nandroid backup. Now I can't connect to my phone with my computer in any way. I can reboot using volume down and go to recovery, but when I try to run the script (I'm on a mac, so I'm running recovery-mac.sh) it just says "error: device not found". Furthermore, if I boot back in to android and try to send the "adb reboot recover" command from my macbook I get the same error. Any ideas?
For the record I have installed the SDK.
Also, I've just noticed that it won't even charge when plugged in to my macbook or my girlfriends win7 machine.

ive only briefly played with the phone. i noticed settings to tell it what action to take when connected via USB. perhaps review these settings, also make sure ADB is turned on in the applications, usb debugging section.
this should at least get you a USB connection in normal android mode.
if you flash the userdebug RUU PC36IMG.zip and reboot into recovery, adb connection should work, otherwise something is wrong and i dont think its the software on the EVO because it seems to work fine for everybody else. perhaps try plugging/unplugging USB, switching USB ports, etc, the usual troubleshooting steps.
let us know how it works out!
No luck, I even tried the exe to reflash to stock (ran while HTC logo was up) and that was a no go.
Any more ideas?
Try a different USB cable. I had a cable suddenly go out on me, and wouldn't charge or act anything like a USB cable should. This was 'not' a cable that came with an EVO... but, worth a shot.

having trouble connecting the nexus s to a pc

i looks like my phone doesn't recognize when i'm connecting it to the PC,
i dont see any option about mounting the sd card on the notification bar, actually there is nothing said about charging over there...
i also discoverd, when i enter the *#*#4636#*#* mode, so at the battery info it says:
Battery Status: Charging
Power Plug: Unplugged
and i'm quite sure its the problem, because the power plug should mention if its AC or USB charging....
i think i've already tried everything!
some other cable, other pc, i've tried flashing another ROM, going back to the default ROM...and nothing! its still doesn't recognize it...
what could be the problem?

next step would be to plug it in via the same usb into another computer. the last step would be to wipe data/factory reset the phone.
I had this problem as well, what fixed it for me was plugging it into a computer running XP. After that, it worked fine.

please help! having the same problem.
This is not going to fix the usb not being recognized issue but is a alternate option. I have a working usb but choose not to use it unless have to.
I use my dropbox account. I download all files there and when I want to flash just sync to phone and flash rom, kernel & theme

at the moment the phone is stocked and is not rooted...so without approach to the sd card from the pc, i will not be able to root it nor flash any ROM....
If you have tried all option then looks like may need a replacement. I don't have the black tie program but there are treads on the return policy

It's working here. Do USB icons show up next to the clock?
Just flashed the same ROM. USB works fine for me.
no USB icon next to clock, tried re-flashing, even ran a super wipe script.
the weirder thing is that with other ROMs everything is working fine, except the 3g on tf101g.
we'll try flashing again.
OK tried re-flashing, re-wiped trough cold boot menu, format each partition seperatly still not working.
does anybody maybe knows which files of the rom are responsible for the 3g working fine.
that way i'll b able to create a hybrid.
thanks for any help guys

Don't wipe with script. Do a normal wipe and install it again.
did it already, with and witout a script.
startong to think that maybe something is wrong with my firmware.
how can i check my device's firmware?>??
it does not show at the About section...
Got it fixed you guys..
so if anyone ever encounter these problems hre the steps i took to sort it out:
Fisrt, symptoms:
1)device isn't being recognized when connectd to a pc
2)device reset it self every few miutes
solution:
reflash recovery bootloader and firmware, i used that zip file:
http://forum.xda-developers.com/showthread.php?t=1552303
then, you will see that the recovery console has changed and that you can't install CM9 Rom.
so what i have done is re-installing the new recovery by Roach
http://forum.xda-developers.com/showthread.php?t=1213723
then format all partitions and re-installing CM9 everything worked for me after these steps
GoodLuck

Restarts from fastboot when connected to USB

Hi, I hope someone can help, because this looks like a strange one. I was not able to find something similar to it.
I attempted to unlock the boot loader and install TWRP. This worked fine. But I noticed that the fastboot icon changed from being a gray CM face to a white one. That was done using using the Toolbox v6.
I later noticed that some android apps are listed as not compatible with the device. So I thought I'll reverse the process. So I plug it in, and clicked lock boat loader, did a restart; wanting to to flash the stock-recovery. My goal was to reset it back to normal.
The issue I have now is that whenever I try to fastboot, it boots into the white CM face, and the second I connected to the computer via USB, it restart. If I try to hold power+volume up again, it just goes black and does not restart. When I unplug the USB, it restarts again.
How can I get it to reboot into fastboot and remain in fastboot after being connected to USB?
Any ideas? The tools listed in this thread:
http://forum.xda-developers.com/oneplus-one/general/index-oneplus-one-resources-compilation-t2843675
require that I fastboot, then connect to the computer. but my very issue is that when it connects to USB it just restarts.
Any ideas?
Thanks,
Maher

It seems to work fine now... I honestly kept tinkering around, it seems to work now. I wish I could offer a better answer. But I am not sure I am able to remember what I did exactly... it was a bunch of plugging and unplugging, restarts, and lots of prayers.
I had a same problem and similar solution
I believe i had found the solution when I change the USB port from my notebook where I had plug my the USB cable from my smartphone.
Same issue here.. I think it is because of USB 3.0. My laptop don't have USB 2.0 hence fastboot or adb wont detect my device, I was using a USB extender which works as USB hub (2.0) it worked many times before now again facing same reboot on usb connection issue.

The charging not being fully functional is not a surprise as SlimSaber is based on Android 4.4 which has problem with the charging for our phones.
The next question is, how are you trying to access download mode?
From what I've heard, a download jig is the way to go. However, I don't have one so I turn off my phone, push both volume buttons in, and, with the other end of the USB cable already in the computer, plug in the USB. Sometimes it doesn't go first time so try it a few times (also can take a couple of seconds). It should bring up yellow warning screen asking if you want to continue. Flash away (assuming you have the right files)!
well i have tried with the button combination like you said , but nothing appears and my computer only says ¨unknown usb device¨ and when i try from the recovery it just reboot.

Yes, the download mode reboot from recovery not working is a known issue. Fixing it would have probably involved messing with the bootloaders so it wasn't done (if i recall correctly).
Since I'm assuming you're on Windows, have you installed the USB drivers? Also, it would be worth trying a different USB cable and a different USB port if you haven't already. Typically the ones on the back of a desktop are the best. If you have a laptop, trying ports on the other side is recommended.
Can you transfer files from the phone as a mass storage device? Does the phone act as if its plugged in when it is (ie is there a notification to connect storage to your pc?). In Device Manager on your computer, could you please list off any unknown devices. If you have linux, the output of the command "lsusb" would be great (if not, don't worry).
