I haven't searched but saying that I've never seen anything like this on Desire or Sensation threads before.
I went to copy something over to the phone (using Windoze Explorer) and when I looked at the contents. I saw the attached. The phone, when I use Root Explorer, and PC agree :-((
For reference I've used Linux, PC (Explorer) and ADB 100% to copy files over successfully before, so no posts about using one or not using one.
All the Filenames etc. have been completely screwed up. Looks like they've been converted to DOS 8.3 format.
I have never ever seen this before. I think I must have run something on the PC when the phone was USB connected. It's about the only thing I can think of, the phone/SD Card/ROM have been 100% reliable so far.
LUCKILY, I made a rare backup of my SD Card yesterday. I usually remember to make one once in a blue moon.
All I can think is to recommend folks to disconnect the USB drive when not needed and do Backups (of the Phone and also the SD Card).

sorry to say this to you but your memory card has gone corrupted.
i know this because this has happened to me alot of times.
this happens if you use unbranded memory cards but this also happens to branded memory card.
the only solution i can give to you is buy a new memory card. and the main reason this happen when you connect your phone to your pc/laptop and use it as diskdrive and take the wire out while transferring data.
the only way to prevent is to use a safe remove usb and than change your diskdrive to charge only and wait till everything is loaded and then disconnect your usb wire.
also if you fornat your memory card it will work but i can tell you this that this willhappen again and this could happen without connecting your phone to computer. so to only prevent data loss buy a good new memory card.

Correct me if I'm wrong but I'm 99.8% sure it still works and he doesn't have to buy a new memory card yet .. I don't know where you get the idea that his memory card is completely dead! It looks like he still has his files, there's just all that junk in there! Worse comes to worst, he has to reformat it which I think may not even be necessary
Anyway, I've had a similar situation as yours and "repairing it" fixed those issues up. I didn't even reformat it, just repaired it. Try that! (Assuming you haven't already reformatted it anyway)

Update:
Card is fine. Just the filenames/pathnames got screwed.
As I said luckily I had done a backup of it yesterday, only by accident. I mounted the card on the PC directly (USB adapter) and deleted / copied stuff back.
It must have been something on my PC I ran as it changed the filenames/pathnames to 8.3. I did run something (can't remember the name) as I had problems doing the copy (SD Card to PC) for the backup. A few (4 if I remember) files were too long, exceeded the 255 limit. I D/L something to see what was the issue, turned out to be some book preview I'd accidently down loaded from the market.
All good and working now....
As I said just a warning that **** on the PC can screw the SD Card on the phone.
Backups (Nandroid and SD Card) are a pain but can be life savers.

i don't mean that he should buy a new memroy card because after formating/repairing it the memroy card will work but this corruption can happen again. if this happens again the onl;y thing he can do is buy another memroy card. this has hapened to my unbranded memroy card which was 16 gb class 10 and it happened atleast 5-6 times. since i couldn't take another corruption i have ordeered class 10 32gb lexar memory card which is working fine and its hasn't gave me any problem since i've been using it
Man that's what happened to one of my other 4 gb cards I don't use anymore. It was kinda funny. I ended up reformatting it, but an odd thing has happened... 3gbs went down to 1.7gbs... I can't even access the remaining 2gbs... no matter what I've tried.
I didn't buy a new card since it works just fine. Been using it as a "mini" file storage card for 4 months and no issues yet. =D
You sure everythings fine? A lot of files on that card read "0KB" which is definitely not a good thing.

It's fine.
Just that something I used, never used it before, to check filename/pathname lengths (due to FAT issues), seems to have converted everything to DOS 8.3 format. The ones that didn't conform to that were created after a phone re-boot.
This was only as a warning that **** does happen and backups, while a pain in the arse to do, can save a load of pain.
Off to try some ICS ROMs now so expecting a load more pain, d/l, CWM, Wipe, Swear, revert back to CM7. ;-)))) Just need to make sure that nothing updates my HBOOT..
Try .. A Full format of the SD Card
You need to do a Full format and wipe of the SD Card .. I have " fixed" a few this way.
I use: HP USB Disk Storage Format Tool - Seems to work for me

Hello, recently my micro sd 16gb sandisk memory card got corrupted. i rescued some files, but i can't format it. it says windows was unable to format. my phone don't even recognizes the card and it can't format it. i tried some other software, but no success. maybe it has bad sectors or something. i don't know.

Sadly, you'll probably have to get a new one. If you try to get it to work, you'll most likely lose even more data! The same thing happened to me only three weeks ago. My card was at most six months old.
Basically, microSD cards are very delicate. As if by fate, I had just read an article about that right before my card went belly up! My new card even has a notice recommending that the card be inserted and removed as few times as possible, for this very reason.
Given this, and given that, for the most part, when they're in good condition they're very reliable, when you start having problems with a card like this, it's FAR more likely that the card has gone bad in some way.
When mine went bad I tried everything I could think of to get it working again. And I succeeded! Trying all different block sizes, I got it to format in the flash drive on my desktop system.
It died again within 48 hours!
I know these things are expensive. Unfortunately, this one is probably a loss. Sorry!

There's no way to tell what the problem is. Basically, though, all problems with flash cards are physical in one sense or another. It's extremely unlikely that it's a driver or card reader error if it was working before. It could be a filesystem error, but if you've already tried to re-format it and failed, well, that doesn't really leave much else.
I don't have your original post in front of me. Did you try formatting it in a computer? If not, then you should definitely try that. (You'll need a microSD to full SD adapter or a microSD to USB.) Also, try doing a full format. (In Windows that means not doing a Quick Format.) A Quick Format just tries to create a new File Table. A full format will try to make a new filesystem. I don't know if there's a way to do an actual hardware format. (Actually, I don't believe that has any meaning on a solid state device.)
Anyway, even if you do get it to format successfully, be very careful what you put on it. That's exactly what happened to me. As I said, after finally getting it to format, it died again in less than two days.
This is about all I can tell you to try. (BTW, I'm a system administrator. ) Basically, and I am really sorry, it's probably dead. I'm guessing they're pretty [email protected]!n expensive in Bulgaria? I know they are here. I almost cried when my first one died!
Although, looking at it in a positive light , when I got my first one they only had Class 2 devices. My new Adata card is Class 6. The difference is amazing. Whatever you wind up doing, my suggestion would be to limit how many times you take it in and out of your phone.
I have a MT3G rooted with CM 4.0.4 using the one click method. I'm getting a new Transcend 8GB Class 6 card. I believe this card come with an adapter to plug into a regular SD slot. To format do I just use my comp to format the card to FAT32? Then load my saved data back to the sd card and load it back into my phone? Is it that easy?

I have the same card (which i will be shortly throwing in the bin due to its unreliablity) and find its better to use the sd adaptor than one of those crappy usb adaptors for micro sd cards (like the sandisk usb adaptor that doesnt work!!)
Just my opinion, but id recomend buying the best make you can when it comes to micro sdhc cards, my transcend one decided to wipe itself clean the other day during a phone call..... a sure warning that its on its way out....

Flash Format is the best way, imo. I've had corruptions before where my pc and fuze couldn't sse the card, but flash format was able to re-format. The demo works fine, and the app costs about 10 bucks. I've had major problems with my 16 g sandisk, but think I figured out how to keep it healthy. Quick tips: 1 Set all default downloads a nd save paths and whatnot to main memory, including PIE cache. 2 Put any apps you use a lot or especially that run continuously (don't forget services) on main memory. 3 Do not put Finger Keyboard on your card, lol. The instructions tell you not to, but this is not a challengr to do it anyway. Guaranteed corruption. Trust me, I did it cuz I'm an idiot. 4 If you use a disk scanner/repairer, and it finds problems, don't just delete the recovered (and messed up files). Open them with notepad, and see what they are. This will give you an idea of the corruption source.
My notebook is not picking up the sd card with adapter? I have a slot on my notebook, I thought it was bad contact, cleaned it and still nothing? WTF!!

I tried that, but my notebook says theres nothing? I also tried just installing the new card and trying to format it with the phone. I formated the card, but it said the card is damaged and it wont do anything? So either the card is just not getting recognized or it is bad? I'll keep tinkering with, if it doesnt work, I'll send it back to newegg.

Oops, sorry, didn't realize you were on android. All I can suggest is searching for a similar app that runs on android, or maybe looking for undelete programs for your desktop. There's probably something out there that will fix your card.

Yh it is subjective and i dont know about recommendations, but im going to think about sandisk, im just very cautious about transcend card after it wiping itself....
i got a replacement xoom and when i tried putting in a memory card in it, the memory card would get stuck. i managed to get it in but now it gets super hot when its plugged in the xoom. any reasons why?
I would remove it very carefully but it sounds like something is wrong. One of the metal prongs/leads/whatever on the card may have gotten skewed and could be causing mischief.
actually i think its the memory card cuz i put another one in and that one works and doesnt get hot. by the way the memory card i tried first i had it in my evo and then a few days back i had a notification saying blank SD card, do you think thats the problem?

it sounds like your memory card has either gone corrupt or is trying to. I would suggest backing up your data on that card. I say this because the memory card that I have for my Droid 2 started doing that as well as *uninstalling* all my apps that I placed on the SD card (saying that the files were never there and such, but when placed on my Windows 7 computer, it would be there,) and then would bog my phone down while trying to read the card. I tested to make sure it was not the phone itself (which it would have sucked!) and the same thing happened in two other android phones, proving it was the SD CARD because other (as well as newer too) SDCARDS I had laying around and none of them had this trouble. Even a SD Card testing APP on the market said "Uh yeah... it's dying!"
Because Android is on a linux flavor, it likes to KNOW EVERYTHING on the card or it just does not care for it and then treats it like a blank card. I would suggest that if you want to try and salvage the card, do a quick format AND I WANT TO MAKE THIS CLEAR: to the same file allocation system (aka Fat16 to fat 16, Fat 32 to fat 32, NTFS to NTFS. DO NOT CHANGE THIS unless you know what you are doing!) Also note DO NOT do a full format because this will write to every sector of the SD Card, and that is really really bad for the SD card as well as any flash memory...
A quick format just goes over the file allocation tables (a table that stores information for where the information is located on the card) and then try it again. If problem persists, call manufacturer of the card or where you got the card originally (evo = call htc, droid 2 = call motorola, ETC) and see if it is still under warranty.

Hello i have a big problem. I installed the ROM coredroid one week ago and yesterday my 32gb sdcard stopped working. I checked it in my computer and it works fine, but when i insert it. It shows not available in my phone.
I tried another sdcard but only with 4gb and it worked fine in my phone.
Does anyone know what happened and what can i do?
The 32gb sdcard is also not recognized in recovery mode.
Is there maybe an electrical damage? But why works the 4gb card?

the sd could have been damaged in the download corrupt file. but will it it give you the opotion to format the sd card
no there are no option to format the sdcard, to only one option would be to format it at windows and put it again in.

There is an option to format your SD. Go to settings, storgage etc..., unmount SD, and you will have (enabled) the option to format it.
Something like that. I cannot see it on my phone right now, but I have done it before.
I think the sdcard is damaged. I can read the index on my computer, but when i try to backup anything then its near impossible. It transfers the files with only 4-8 KB/s instead of 10-20 MB/s.
Damn i hope my seller will replace the card. Its only 2 month old.
Never thought that a sdcard is possible to get this cause.
If you remove the sd card alot you can damage the contact points at the bottom of the card and it wont work . Scratches from putting it in and out over time wears the gold contact points out.

I had the same thing happen. I was able to finally get everything off my card, though it took a long time because I had to transfer one folder at a time or the card would just act like it was removed if I tried more. Anyway,once I finally got everything backed up, then I reformatted via card reader on my computer, then I put it in my phone and reformatted again. The I was able to load everything back on, and it's Been fine ever since. All I can figure is it for corrupted when I was shooting video while the battery died a couple of times. But like I said, its been a month since that happened,and all is good so far.

I never took it out of my phone. Its stopped working while i used it. It showed me the message sdcard is unmounted and then it was gone. I copy now my important data from sdcard, but it takes very much time with 30 KB/s.
Maybe a format will help.
My sensation gives me "removed sd card" or "damaged sd card" message everytime the phone starts to get hot a bit. If i use the phone too intensely, the bottom part (where the sim and sd cards are located) gets really hot.
Then the only way i can get my sd card to reconnect again is to wait until the phone has completely cooled down and restart it.
It does this on many roms so I don't think it's a rom issue. I was just wondering if anyone else had the same problem and if there's a fix for it. It's starting to get really annoying.
I'm currently on firmware 3.32 and running AOKP but it did the same thing on CM9.

Did you tried to format SD card ? If you did and still have this issue, than only remedy for you is to get NEW SD card mate

I haven't tried formatting it yet... i will try with another sd card first to see if that may be the problem (the one currently in my Sensation is 32gb and it's going to take me forever to back up everything and put it back) and if so I'll reformat it. Fat32, right?

You are correct, this need's to be done mate, so you have to put up a bit with data transfer If you have class 10 as i do it will take up to 30 min each direction ( i have 28 gb of data) Thought i think that card is culprit) ... Time will tell.
BTW i always format sd from recovery
Without inspecting your hardware (that sounds dirty) I can't know for sure but it's a possibility that the excess heat is causing an already damaged SD Card to expand just enough to screw with it. When it cools, whatever is damaged probably shrinks back to normal and all the electrical pathways in the card re-align properly.
That shouldn't happen. An undamaged SD Card will still expand with heat just like anything else (not noticeably, but expansion still occurs whenever anything gets hot) but it won't cease to function properly.
Remember you're dealing with REALLY tiny electrical pathways in a Micro SD Card. Even slight expansion causing misalignment could screw everything up.
In other words, like everyone else said, get a new SD Card.

But what's funny is when the sd card heats up I can still read it with my computer...
I just tried with another sd card, rebooted a bunch of times to get the phone to heat up a bit (if I would do that with my 32gb it would disconnect) but everything works fine. So at least now I know it's my sd card and not the phone itself.
I'm backing up my data to at least try reformatting it before getting a new sd card (i think i only have a class4 so it's going to take a while). If I start using a new sd card i'll need to transfer my stuff anyways.

Thanks for the tip. Also I forgot to mention that even when I can't access the sd card from the OS, I can access it from recovery, so I doubt that the card is actually damaged. Hopefully reformatting it will fix the issue, I will let you guys know.
Just to update on my sd card situation, I started to back up my data to reformat it, and I cleaned it up a bit by removing some unused folders and files at the same time. All of sudden no matter how much the phone heats up, the sd card doesn't disconnect anymore...
