Here is a modified stock Evo 4G 4.24 GB SystemUI.apk that has the quick settings enabled. This is my first mod post so please forgive me if I missed something.
I am posting this cause I have looked far and wide for this and did not find one specifically for the Evo. I did find several that worked on the Evo but didn't have the 4G toggle.
This apk is straight from the HTC RUU as all the parts were there but disabled. Thanks to SK806 for providing some helpful hints to get me rolling.
Update:
*Fixed FC's on 4G toggle
All toggle are now enabled, and a screen shot added.
Task manager and Recent apps are still off cause I didn't want them.
The order of the toggles in the list have been changed to Brightness, Wi-Fi, 4G, Mobile Network, Bluetooth, GPS, Wi-Fi Hotspot, Auto Rotation, and All Settings.
Plans:
Flashable zip
Add Transparency to the lists
Add an entry for the flashlight (cross your fingers and toes for this one)
I plan to make a flashable zip but for now the apk will have to do.
Just push this to /system/app and makes sure the permissions are set accordingly.

I'd like to say it's the same but, this is not the case it's a port. I modified the origonal RUU files for the Evo plus I have plans to enable other options within it that are not found yet.

I sure can, but I have already completed that step and hope to post the final apk shortly. I'll post the smali edits in the op in the same stroke.
Just FYI, ca1nes version is not a port, he just got the code from someone else(mrluje, I think) and took the 4.53 and added the code to activate it, I can confirm this.
The smali changes were simple, one line of code. Good job tho

Sorry I didn't decompile ca1nes to see, just based what I said off his verbaige in his op. Either way now that my revised apk and screenie is up you can clearly see the differences.

Check my post on the other thread for instructions on the transparent background. I don't remember the status bar. I think it may be in framework. Been a while. I was thinking about figuring that out again. However, I would want to fix all the status bar icons, and that was a pain.

Been looking into these two items already and both from what I can tell so far is that they are both controlled in the systemui.apk classes. I need to fix an issue first thought as it seems the 4G toggle broke when I changed the order. It worked when I first tested the posted build, but I tried using 4G over the weekend and got Fc's. So gotta fix that first, let me know if you guys get the same (if using my build). Gotta get this fix cause its the only things keeping me from using GB as my main driver.

Works perfectly. I have my quick settings and my 4 in 1 boot and now i'm coo. All that other stuff is cosmetic. i'm all flashed out until custom 4.53 roms come out.

I can make a separate zip that will have both quicksettings and recent apps, though I am done for the night, so probably in a couple of days, I'll have it out, rest of the weekend/week is going to be busy.
I've used widgetsoid to replace pcw. Here's a guide on how it works.

But I always use theme stuff. Would it be hard for you to identify what you changed in systemUI so I could pull it out and just drop it in systemUI of theme I use? Or is it more complicated than that?
Also, does this affect the PCSCII that pops up when you plug into USB?

Here are some screenshots from my Swagged Out Stock ROM (attached below).
Only issue I faced was that the SystemUI.apk didn't work on it's own. (The apk was missing a META-INF folder, is that normal?)
Anyways, I re-enabled transparency on your classes.dex (after decompiling of course), pushed the edited classes.dex to my SystemUI.apk and got it working. (Although the transparency step is not needed). I have attached a working, Swagged Out SystemUI.apk (push it with Root Explorer, ES File Explorer, ADB, it's up to you).
From my testing, I thought some of the settings were not working at first. But after some time, they all started working (nice!).
With your permission, I would like to include this MOD into the next release of SOS (adding in the previous Swag as before, of course). All props would be directed to you (duh).
Also, for future reference (when you have the time), could you throw together a quick tutorial of how you did this? Like, which smali or xml's you edited and where. That would be great for future OTAs. Unless EOL means EOU(pdates) for the EVO

Sorry for the question.... but in the op it states that Auto Brightness and Auto Rotate are available. When I installed I have everything but those two on my quick settings bar. How can I get those added to my quick settings bar? Did I miss something? I am running the Stock Rooted deodexed 4.53 rom posted by NinjaWolf.

good catch, yup those aren't in the quick settings list yet... I'm working on porting as many options as I can find. though I did also note that I didn't list 4g as an option which is actually in there, updated OP... thanks again for letting me know about my typo.
Okay so I am slowly diving into theming and decided to look into some XML edits. My goal is to make some edits to the status bar and move the clock to the center, notification icons to the right, and signal and battery icons to the left. Here are the steps that I took:
1. Using APK Manager, I placed the SystemUI.apk from Humble 5.0 in the place-apk-here-for-modding folder
2. Selected the apk using TWframework-res as a dependency and then decompiled systemui.
3. Navigated to projects/SystemUI.apk/res/layout and clicked edit on the status_bar XML file.
4. Went to the line containing com.android.systemui.statusbar.IconMerger and changed androidaddingLeft="6.0dip" to Right and changed android:layout_alignParentLeft="true" to Right
5. Changed the line containing android:id="@id/statusIcons from androidaddingRight="6.0dip" to Left and changed android:layout_alignParentRight="true" to Left.
6.Changed the line containing com.android.systemui.statusbar.Clock from androidaddingLeft="6.0dip" to Center and left the rest of the line alone.
Can anyone with more knowledge of XML edits help shed some light on whether or not this will do what I am looking to accomplish or if there is more to it than making these little changes? I'd eventually like to find a way to move the status bar to the bottom of the screen similar to the Nook Color but I gotta take this one step at a time, haha. Any insight is more than appreciated!

You need to change a lot more than just that. Give me a few minutes and I'll link you to a guide. I was able to use this guide to get the following changes:
Centered clock
Icons on the left
Notifications on the right
Only problem with my results is that the notifications overlapped the clock.

Yeah. The only way around this is to give the notification icons a background color instead of having them just be transparent.

Yea I figured it was much more involved. Thanks for the link spc, I'll post up a screenshot once I figure all this out. Once I get the hang of this I may even try to get notification icons to work using Statusbar+. I think its very clean looking and, since I'm going for the WP7 look, it's a WP7 status bar but doesn't support notifications.

yeah thats about as close you can get to center clock mod without diving into smali
the mod used in tsm parts and other apps like that is different and puts the clock in the center in a different fashion and doesnt overlap, but will still be present when u use the pulldown

What it has:
-Ics styled notification bar(almost done ran into a hickup with the images)
-Roboto font
-Ics animations
-Ics bootanimation
-New signal meter
What needs to be done:
-Replace the rest of the images in the framework and systemui that are not the same as the ics images
-possibly theme some of the apps
How to install:
Transfer the theme to the root of your sd-card.
To install you must have clockwork recovery dont need an unlocked bootloader to get it go here http://forum.xda-developers.com/showthread.php?t=1390771
If you already have it or when you get it do a backup of what you have im not going to be held responsible for your lack of backing up and you not wanting the theme anymore. Or your bricking, breaking, exploding, impoloding, or someone stealing your device.
After BACKING UP!!!!!!! Install the zip it has worked for me three times(wanted to make sure it worked) reboot and enjoy

In version 1.0 i fixed the unreadable menu bug looks less like ics now but is easier to use. I'm waiting on feedback from the developer of apktool before I go any further with this theme this is the final build until I figure out why the framework wont decompile and compile correctly.

Installed, noticed that some of your battery icons were the same for say 50 to 60 and when playing music it is difficult to see the song name in the pull down notification menu, it is black and the back is very dark transparent, might just be me though. Great start though, like the transitions.
Thank you for your feedback battery is more accurate now and going to fix the weird music and download notification thing tomorrow when I'm less inebriated lol

Just flashed it over. Looks pretty good! The menu labels however are black on black, meaning I can't read them. When I go into Gmail, select to change the labels of an email. A list pops up but I can't read what any of them say.
EDIT: Also the menu that comes up when you long-press the homescreen can't be read.

Edit nvm I know what you're taking about was thinning the home key lol. The issue is I need to change the pop up window txt color and menu txt color I think.

Will this mod whack my currently installed EQS? From what little understanding I've attained I expect it will.

I have the EQS installed from the latest IC..THE ONE WITH ONLY EQS SETTINGS...NOT MIUI BATTERY BAR, ROSIE SETTINGS...The framework used for this mod was the framework from IC with EQS installed...I pulled the framework-res.apk from system/framework after install via root explorer..
In short, Nandroid and try.

Thank you guys for all the support!! I will have an update out ASAP..my intent is to only add color to the bland icons found throughout sense, while still maintaining a stockish feel...HTC, WHY AREN'T THESE ICONS COLORED ALREADY?!?! LOL

I'm expecting to do a lot more work to my theme, in short I will also be adding a donation link, cuz all of this themeing is making me quite thirsty
Edit: and to those waiting for EQS update should be out in 8-10 hours..so lay off the f5

Would you mind providing me JUST the modded images (again, I do not intend on taking credit, just providing an alternative installation method)? It would make it MUCH easier to convert to a TIWILI install option if I didnt have to crack open the apks and find all your colored images (especially as you keep adding more of them ).

Already had a donation in mind.
Update 3rd August 2012
Theme now is just one version combined layout style of previous 2 versions
includes all the minor changes from previous update(touched up data icons)
removed search icon because not working
download and screen shot at bottom of post.

This is a mini theme I guess. It is a port of the sensation rom systemui.apk and the battery charge anim works just like the original
It is only the systemui, criskelo-res and quicksettings apks and it is 6 toggles on screen because I don't think the 8 toggles suits.
Suits best with a blue or stock framework.
It is not fantastic and may need some touch ups but is at a point I am happy with and ok for now
Attached is two versions one with carrier text and one without (for now unless I can get it to look normal with the carrier text)
the pictures show the difference.
This is working on 4.0.4 LRK Classic rom but should work on other 4.0.4 roms too but the usual make a backup and flash at own risk
Both are cwm flashable

For the one with Carrier Text, are you able to add the additional icons beside it ? E.g. lockscreen, etc ?
I Haven't tried yet. Ill look into it when get some time and whether I can get it to look any good if i can get it to work.
I'm trying to add carrier text back to the one without and make it look ok, this is the best that I've done so far, i just can't centre text so its going to stay on the left for now.

Thanks.. one comment. The Data logo is a bit low. If you can move it up a few pixel to centralize it, it'll be perfect !

How did you get that "Ongoing" list of apps to appear ?

Yeah haven't got around to touching that up yet but will get to it, just wanted to try sort something out with the carrier text first.
Its an app from play store called elixer2 its free and I only use the status bar widgets cause they are very handy

I have touched up the data icons a little, raised the data in and out icons and centred. This does bring them a little closer to the wifi icon but I dont think its too bad as looks better this way now.
Unfortunately I have not been able to get a carrier name on the no carrier version and be fully happy with it and with getting the lock, recent etc icons onto the carrier version, I can not get anything that is stable enough for daily use, so going to stay as is for now sorry.
Disregard the green text and date in the screenshot as that is from my framework-res. If you stock or blue framework-res the text colours will remain the same.

Thanks. Feedback:
- No carrier version - 3G, H and H+ are perfectly located, but Carrier version icons are misplaced
- No carrier version - Clear is really tiny
Looking good mate, nice work.
However I've tried both of your nocarrier zips and single click in the search icon does nothing. I'm on classic rom v3. Anyway we can fix this?
Also if its possible to make the 'clear' text just a bit larger so it would center perfectly in the button.

Hi, are you using the no carrier version? If so does your search button work on single click? Thanks.
working on getting the lock etc buttons on the systemui with carrier text so there will be just one version which has both things. Just having some stability issues with it hoping to niggle out so can use it.
I have fixed the data icons on the carrier version (sorry I mustn't of edited all the images needed my bad) will upload the fixed version later when get a chance.
As with the search button I have noticed before the theme it didn't open anything but I have just themed criskelo's systemui.apk so if anyone is using criskelo rom can they see if it works for them, as far as I understand in the code it is pointing to a search application to open but I am not sure if it is executing the command or just doesn't have the required search app installed. I will try and play around but can't gaurentee I will be able to fix that issue as it is there from the unthemed version (for me anyway), may have to remove the icon if can't sort anything out.
One more suggestion, the Wi-Fi logo look very low res, can you improve in that too? thanks
