I have this problem that keeps repeating itself.
I have been using the latest BiffMod Rom for a few weeks, all has been fine until I moved some apps to the MicroSD card via the inbuilt application manager.
The phone works fine for a while and then one day, nothing. the phone doesnt want to boot or anything. After some investigating it turns out its the MicroSD card that is corrupt.
If remove it from the phone, the phone operates as expected, If I insert it, I either get freezing, or the safely remove SD card message. The card wont mount.
I have tried the card in a Windows PC and it will read the Fat32 but freezes after a while.
Now this is not the first time this has happened and the pattern is emerging that it seems to be when ever I move apps to the SD.
Has anyone else had similar problems, I have gone through 3 of the same card in about as many months, luckily I have a bunch of cards from work. But its super annoying.

Well, for one, Biffmod moves apps2sd automatically, using scripts and when you do it manually, you are messing with the mod. So, thats where you messed up.
Secondly, I too messed up a 16GB Class10 SD card but, was able to fix it by reformatting it in ubuntu then again in Amon_RA recovery menu. I originally had it formatted 1024MB ext4/96MB swap...when I re-parted it, I had to change the sizes for it to work. Now, its 512MB ext4/32MB swap.
Third, a guy (MrBurrito) gave me this script file he made to try and fix the card. You copy it to your sd card, flash it in recovery, wait 10-20 seconds after it finishes and, then reboot. It didn't work for me but, I'll attach it, maybe it can work for you.

I've been using Dexter's froyo v1.3 for a week or two now, with basically no real issues. About 3 days ago or so, when I powered on my phone - you know how the sd card symbol appears in the status bar right when u turn on phone, it's loading sd card - so I see that symbol, but unlike how it usually is, it doesn't go away after a second (after it's loaded). So I pull down status bar and the icon says "checking for errors" and it doesn't go away at all. SD card just won't load. Oh I should mention -just remembered - I turned it on and I got "error with sd card, you may have to reformat" (or something like that) right before and then it just changed to "checking for errors" after I took out the sd card and reinserted.
Anyways, so my SD card isn't loading, right. I figured it might have something to do with the fact that I was using the SD card that came with it which is low class (2). I tried putting it in an SD card reader and put into my computer and it showed up and worked with no issues. But it just wasn't working on my XT720. So what I did was, I happened to have an extra micro SD card lying around (this one class 4) and I put it into the XT720. It worked just fine. That was a few days ago.
A few hours ago, I was using my camcorder on XT720. After using for quite a while I got "sd card not present, sd card must be present to use camcorder". I left the application and went to settings and my sd card was registered as present. I then went to gallery but I got the same "sd card not present" thing. My phone felt very hot at the time, which I assumed because I was using it for a while. I figured it might be an issue of overheating causing weird things. So I decided to power off my phone for a while and see what happens.
Upon turning on my phone, I was greeted with the SD card icon in the notification bar and upon pulling down; "Checking for errors". Now this new SD card won't load.
This is a really big issue for me, because I really need the SD card for stuff. Both SD cards seem to work fine in SD card reader on PC. Please let me know how I can fix this...

I'm going to try some of the solutions people posted, but problem seems unfixed for majority of users .
Gaaaaaah. My Android experience has been plagued with far too many issues....
What sucks is that if I want to flash back to stock to try to use warranty, I'm going to have issues if I need an SD card....
It seems you've already determined the issue, you have excluded the SD card(s) as the source of the problem. This leaves one thing - your phone is defective.
Flash it back to stock and send it in for service.
This issue has absolutely nothing to do with what software is on your phone.

You don't need the sdcard to flash a stock SBF with RSD lite.
I kept having same problem. If u are using move 2 SD or anything like that select auto and not Justin external I had to reformation my card about 3 times b4 I figured it out. Dexter posted somewhere not to use these apps.

The only problem I've had with my sc card is that most of the time after my phone has been rebooted, the sd card will show up as not present once the phone has completely finished booting. Sometimes I can just reboot again and it will pick up on it but usually what I have to do is shut it down, remove the card, start it up and then once it indicates the sd card is not present I can reinsert and it loads just fine. I have an 8gb class 4 that came with the phone.

I tried that once (it was something mentioned in that link I posted) but it didn't work. I'll probably try it out again before attempting to use warranty.
I would also try reformatting the sdcard, just because Windows can read it doesn't mean there aren't issues with the card.
K, so I put it in SD Card reader and into PC and began transferring all the files on the card to a new folder. During this process I got a pop-up a bunch of times telling me specific files were corrupted and unreadable and couldn't be transferred (mostly music filer for the most part). I then formated the card to delete the remaining corrupted stuff. Now put into phone and it registers as being there and now things like camera are working.
Gonna have to see how it fares when stuff are actually on the card, but for now things are looking good.

And after transferring some music on to it, it still seems to be working fine. Needless to say, I'm quite happy I don't have to go through the hassle of flashing stock and then trying to get a device exchange for warranty...
Though I am slightly worried that the device itself will corrupt things again (since my other SD card also worked fine for a few days). But I did also do a complete factory reset so perhaps there was some app causing issues that no longer will... Guess we'll see what happens.
I had problems with my stock (Cincinnati Bell) SD Card doing a similar thing -- showing up as no card inserted. Sometimes rebooting did the trick in fixing it. Then my phone fried and quit working. I thought it was related.
After I received a new (refurb) from Motorola as a warranty replacement, I used that same card and was getting the same error. So I upgraded to a new 32GB class 4 card and still got the error. I upgraded to Dexter's ROM and it still continued. I'd keep rebooting (three to four times sometimes) and eventually it would go away.
Then I decided to pull everything off the card, reformat & give it an ext3 partition using the method in the video tutorial in these forums. I haven't had the issue since -- it's been about a week now.
All of a sudden apps on storage card fail to load.
After a few reboots they reappear at random, but disappear again on reset.
Did not have that problem with stock, but after flashing Infused 1.14 (and on) the issue appears even with the (rooted) stock rom.
Any ideas?
I am trying to determine if I need to return and replace the phone before I lose the option.

Answering the question by myself, in case others encounter the same issue.
The external card had something corrupted, and when the system was mounting it, all mess ensued.
Backed up all the data and reformatted.
System appears ok for now
I just had this same problem or at least something very similar to it. Initially I thought it was the 32GB SD card being too big since I read Android had a 16GB external card limit. But those posts were from a year ago and when I searched deeper I read that 32GB cards were being use without size problems now. There was one issue that seemed to affect some people and that was that android could have problems formatting a card that big and that's how I installed it in my Infuse initially- I let the Infuse format it, then popped in in my Macbook Pro (in OSX) to copy over the contents from the previous 8G card. I believe I have hidden files set to show, so I should be copying over everything- though, could this method of upgrading the card be a problem too?
It worked for a couple days, but last night during a reboot I tried to access the app tray and didn't know the card was still being scanned. It froze and I couldn't get past the secondary media scan after many tries. I didn't realize that apps were being automatically installed to the card.
Anyway, I was gonna return the card, but after reading your post, I'm thinking again about corruption due to poor formatting as the problem, so I'll try it again when I get home. Thanks for your post. I really want to be able to use this larger card so I'm keeping my fingers crossed this will solve the problem.
BTW, how did you format the card? I have Win7 x64Pro on my MBP as well if I need to use windows, but did you use FAT, FAT32, ExFat? I'm still a 'droid noob ;P. Thanks again!

I have motorola atrix 2 MB865 with 32gb sd card. It was working fine for a long time. I had pattern lock and my kid put multiple patterns and got it permanantly locked. There was no way of unlocking it other than resetting the phone.
I did that, since then there were lot of problems related to sd card. Often my sd card was shown as damaged sd card in phone and i need to reformat it but in pc it never showed like it. Once reformatted it worked fine for a few days.
Another problem is that while I am shutting down the phone it gets stuck on that process, as soon as i remove the sd card - it shuts down immediately. (its FAT32 formatted)
I am not able to understand what problem it might be. Have any one of you experienced such a situation and solved it. I would like to fix it as soon as possible. but out of choices. Any leads would help.

As the title says, something seems to be up with my SD card. I figured I'd nose around here before going out to pick up a card reader so I can mess with the darn thing on my computer.
Anyway, before anything else, I'm going to say that I've never had any issues with writing to my SD card since I picked up the Z3--neither on Kitkat nor on Lollipop. I don't even remember having to use the platform.xml fix.
Just today, however, I suddenly got an error when trying to copy some files to my external SD. I was more than a little surprised since, just a few days ago, I was moving files back and forth between the card and the internal memory without any problems. I tried backing something up in Titanium next, and got the infamous insufficient storage space error. I found it really weird but, since it wasn't anything new to me, I tried applying the fix, which didn't solve the issues. I also tried the Play Store app and FolderMount. FolderMount seemed to work so I uninstalled it (which you should be able to do, right?) and restarted my phone, to make sure whatever fixed things stuck. Aaand I ended up right back where I started.
Next, I thought of unmounting and remounting the card in Settings. What I saw there, though, was that the system wasn't detecting the card as mounted at all. I tried using the mount option but all I got was "Preparing SD card... " and nothing after that. Despite that, I could see and access/use everything on the SD card through ES and other apps. I pulled the card out and stuck it back in, and it finally started showing as mounted in Settings, but then everything else (ES and other apps) was showing that there was no SD card. Restarting the phone after that resulted in the original problem.
I've had a number of cards die on me in the past and, so far, I'm not getting any of the symptoms of that yet. If the card is indeed on its deathbed, I wouldn't be surprised either, as it is several years old. If I can, though, I would still rather forego the expense and trouble of getting a new card, so I'm hoping someone can point me in the direction of getting this sorted out.

I've been having some problems with the SD card also, and I suspect XZDualrecovery to do some weird stuff to the SD card..
I've been having problems with the SD card being 'damaged' according to the phone. However, if I take it out, and put it back in, it works like a charm again, until i reboot.
This error seems to coincide with XZDualrecovery, seeing that XZDR doesn't work anymore right before the 'damaged' messages.
When reinstalling XZDR, the error doesn't appear after reboot, and the thing is fine again for some time.
I really don't get why this happens though...
Card is a Sandisk 128GB Ultra MicroSDXC A1 card.
Card is formatted exFAT.
Card can be mounted, opened, and read/write with no problems via my laptop when plugged directly (SD slot).
Android (LG G5, LineageOS 15.1) says card is corrupt, is trying to make me format. Card initially set up as "Portable Storage".
Took FOREVER to move music onto this card, as artists had to be moved one at a time or transfers would fail, so I'm trying to avoid formatting.
I've tried rebooting the phone, repairing the file system using TWRP Wipe command (fails with error code 1), and mounting with a clean eject via Windows 10 on my laptop using the SD card slot. Nothing has made an impact, stupid phone still won't allow me to use the card.
If anyone has any ideas why this is happening, or how I could potentially fix this without having to wipe the card, I would appreciate it. This thing has been a PITA since I got it, I thought it would be awesome having such expandable storage on my phone (I'm a weirdo who doesn't use streaming services) but I've had nothing but problems so far. I have to be careful with how I transfer files onto the card or the transfer will take an (appropriately) long time, act like it works, but then the file just won't be there. Now I've finally gotten the card loaded up, and after a routine phone reboot a few days ago, the Android OS is trying to say it's corrupt; This is also making me worry that this will continue being an issue in the future.
Running "chkdsk E: /r" on my laptop is revealing a couple minor corruptions inside the directory, and volume bitmap. It's hanging on verification/completing now.
Is it possible that I have a bad SD card? This thing has seriously done nothing but suck since I've been trying to use it.
Edit: Unless someone tells me otherwise, I'm pretty sure this is a bogus/defective SD card. I simply cannot believe that flash memory would perform this badly. I'm going to return this card (bit of a PITA since I ordered it online) and buy another one locally.
