Turn off phone, hold volume down, the power button, and the camera button at the same time to enter the recovery partition, and recover the phone
Use Volume keys to move among options, and the Home key to select
reboot system now
apply sdcard:update.zip - Manually update from your SD card
wipe data/factory reset
wipe cache partition - Didn't do anything for me

An easy way to get your MSL is by using the "getprop" command in ADB or with connectbot using a connection to "localhost". It's the 6 numbers next to the label "ril.MSL".

Confirmed. My default is also "Unknown", try "CDMA only" and toggle airplane mode off/on, then let us know what/if anything happens with TWS.

Can't say for sure. I've been using it but I'm already rooted. I can say however that it appears that when I can't get a lock, using the reset GPS command fixes that right away - now if I could just get put that in an app I'd be happy.

I'm REALLY new at this so can you explain this in even more dumbed down version?
Dumbed down version, step by step...
Search the market for 'connectbot', and install it.
Once installed, open the program and hit Agree, and Next until you get to the black box with the text bar at the bottom.
On the dropdown box, instead of ssh, choose local, then enter a Nickname (as far as I know, can be anything) and hit enter.
You will then get a command prompt... enter the command 'getprop ril.MSL' (without the quotes), then hit enter. A bunch of information will then be listed.
Scroll up through all the information, until you see [ril.MSL]: [######], where ###### represents the actual MSL code for your phone. (updated info from panthar adding ril.MSL after getprop thanks)
The six numbers that are then listed is your MSL code.

It appears HTC and Sprint fixed this 2.2 ## code issue. The 2.2 fix listed here should no longer be needed unless the ROM you are using has an old 2.2 Sprint update. BEFORE APPLYING this update check and see if the ## codes work. This little test might prevent problems.

The "fix" for 2.2 only works on rooted phones for now. I will update if a fix for non-root becomes available.
Thanks everyone for the fix! This is what worked for me. Download old EPST here (thanks to aporon for providing apk). Mount /system as r/w with root explorer and copy or move EPST to /system/app. Then install and reboot by clicking on EPST that you just moved to /system/app. Test with ##775# or some other code. It should work.

Yes, Froyo and custom dialers do not support these codes. As you can read in my original post I'm working on a fix for that issue.

I just tried 778 and it didn't work for me. I can get a lot of these to work, but not most of the unverified ones. Do I need to go to stock to get them to work? I figured if a few of them would work most of them would.
I just updated the post. It now accurately displays my results. I am going to flash a stock rooted rom and try the unverified ones again and see what results I get. It is being reported that some work only on stock. It's weird that I have problems with so many of them, but others I do not.

Anything to format those damn SIM's? I have an unactivated one with undeletable voicemail (force stopping Phone works only until reboot). Factory Reset from Settings didn't work, not willing to try that again. Only inserted it to get rid of "No SIM Inserted" notification, which is apparently harder to remove. I also remember getting access to more free games with it in for some reason.

I have a lg rebel 4. 8.1.0 oreo. The phone carrier is straight talk. I've searched Google up and down and haven't found a single secret code that would work. Does any1 know any that will work? Like the basic androids codes are no good like *#06# IMEI info code doesn't work and is suposed to be universal
