Hello dear experts,
I've searched the forum (and many other websites) and found some useful information but nothing 100% like the problem I've got with my Note 10.1. If anything, it's made me more confused.
So here's the story. My Note was running the Gnabo Rom for quite sometime. It suddenly gave up a week ago. The tablet was on, I left it on my desk, and when I grabbed it again two hours later I saw the Samsung boot logo and the tablet was quite warm (the back side). Well I'll reboot it I thought but it got stuck at the logo again.
What works: the download mode (and connects to ODIN on my PC fine) and the recovery (CWM). After reading about recovering the system, I first tried wiping the cache & dalvik partitions etc. But CWM gives me errors: can't format and so forth. the CWM also shows can't access recovery.log when I boot into it. I've tried restoring a backup from the external memory card - the tablet restarts after checking the MD5 checksum. I've tried wipe data/factory reset. No luck. I've tried formatting partitions but it gives errors (system - can't format).
I've tried flashing a stock ROM from sammobile (4.4.2) through ODIN. It gets stuck at "NAND write fail".
I'm no expert but it looks like a severe file system damage.
I've also tried ADB to look at what's on the mmc, hoping that maybe e2fsck would be helpful. No. It showed a "short write" error when I tried to check the system partition. Running fsck on other partitions just gives a bad superblock error.
Dmesg shows some errors when reading the flash memory ("timed out sending SET_BLOCK_COUNT command").
Now this is where my knowledge and ideas stop. I'm worried if it's the NAND that has said goodbye. Any ideas? I'm happy to run some more testing if more information is needed to diagnose.

I think my phone is partially bricked. I know that sounds weird, but here's the situation:
I was able to unlock my bootloader (faced multiple write errors using fastboot but it did unlock eventually), I can't reflash using Odin (it fails Error is "Complete Write Fail"), when I tried to format some partitions using clockwork I get:
E: format_volume: make_extf4fs failed on /dev/block/platform/s3c-sdhci.0/by-name/system
Error formatting /system!
No linux newb here, so I'm guessing my internal partition is fubared..
Here's what's funny... I can flash a rom and boot into the rom usually on the first try. If I boot into the rom, it runs fine until I reboot the phone. Then it takes another *LITERALLY* 10 times of pulling the battery and rebooting the phone because the phone sticks at the Google boot logo. I can boot into recovery, fastboot, no problem.
As my phone sits right now: I can flash clockwork, bootloader unlocked. I'm having to try the NAND restore to 2.3.1 to get it to work. I rebooted the phone multiple times, no problems but then after a phone call...get a notification that the sd card was suddenly removed and now I'm back to square one..
Does anyone have a clue as to why my phone decided to unmount the internal storage and why I can't get this thing to work correctly 100%?

Btw, I usually can't mount the USB storage using clockwork when I get these errors. I have to keep rebooting the phone until I get lucky enough to format the /sdcard, then try and mount it up to my computer.
Just rebooted the phone, flashed /system, and mounted up the USB..
Can anyone explain this??

yup, this ones explainable. classic symptoms of a ns with its internal sd card dying/dead. it happens way too often unfortunately. luckily its a fairly easy and inexpensive repair, if youre not under warranty.
Any direction on how to fix it while not under warranty?
Or take it to tmobile?

check out any local mobile device repair shops. you could probably do it yourself if you dont fear taking the phone apart, it should be to difficult of a repair. its just a matter of buying a replacement(ebay?), taking the phone apart, and replacing the sd.

I think its:
Samsung KB100D00WM-A453 memory package and S5PC110A01 1GHz Cortex A8 Hummingbird Processor.
I'm going to try to find another nexus s mother board as a whole, that's my only option.

i seen something a few weeks ago online on how to replace this...it was a video..i just cant remember where i seen it!
I ended up just purchasing a new phone on ebay.. If I find a nexus s with a cracked screen, I will swap out the mobo.
You can check whether there are blocks flagged as bad by FTL (Flash Translation Layer) in recovery. Check recovery.log file (it can be, the bad blocks will show up after formats).
Hey Everyone. First of all I would like to say that I've had this device for about a year and a half, but it bricked? on me in only 4 months. I've tried countless methods to fix it but no go. Then as time went on I completely forgot about it. Now I don't have my sd card that I used for it any more.
The problem? One day I accidentally forgot to charge my tablet and it died on me, so I charged it right back up and turned it on. Everything went well, I got through the logo screen and everything, but then it just hangs there right before I hit the Home screen where you can do your code and everything.
Methods I've attempted:
I've tried going through CWM recovery to fix the issue but then I notice that it says near the bottom as it loads "E: Can't open/cache/recovery/log" and it has the same "E: Can't open/..." for everything. Can't mount, wipe cache, wipe data, restore, I have no backup (stupid right), can't do anything. Also, when I do attempt to wipe data through CWM I notice "E:Can't mount /dev/block/mmcblk0" if that has any special meaning.
I've tried flashing stock firmware through odin multiple times on different usb ports and different computers.
I've tried flashing PIT file (I'm not really sure if thats how you're supposed to do it but yea, I flashed the PIT file through odin by itself and with stock firmware)
Misc. Info: I did have a custom ROM installed(can't really remember which one), I also have the app2sd installed, the only thing that struck out to me when my tablet went out is that it received an OTA update for 4.1.2? and for some reason it went through even though I never intended to update.
I'm really, REALLY, open to any suggestion that could possibly help fix this tablet. On the other hand if the cold hard truth is that its gone forever or that I simply just have to take it back to Samsung (which I highly doubt I'll be able to), then please by all means let me know.

Did you try flashing a different recovery again. CWM? Also after installing stock through odin try a factory wipe. It would probably help to know what your last rom was.

Hi team,
Wondering is someone can assist or point me in right direction for fixing my wife's Razr HD. ( Australian Telstra GSM )
Issue - Currently phones starts goes to OS , however all andriod apps / core services are stopping making it unusable, usually phone will start ask for pin and within 30 seconds all apps and core services stop ( with error messages etc ).
So here is what happens and i have tried.
1. Tried uninstalling apps ( eg whatsapp / facebook etc .. ) it uninstalls sometime before the freezing / errors - but once you reboot those apps come back as there never uninstalled :|
2. Erase all data it never works it lets me press the button , but on reboot come back to original problem OS.
So i decided i should root / install custom rom or even stock rom ).
1. Boot Loader - Unlocked ( did that on motorola website no issues )
2. Tried Installing customer recovery in fastboot ( it flashes , however it goes to original recovery dead gread andriod with red traingle with belly open !, after flash i have not let it boot to OS still the same issue) - i think it has original-recovery.p file in system i cant delete it or rename it ..
3, Rooting - tried rooting no luck .. i can connect via ADB however system is read only ( doesnt let SU , busy box etc to copy or execute at all ) - tried motochoppper/ saferoot no luck .. it cant write or copy or exceute SU.
So i tried few other extreme measures as below..
fastboot erase system or -w
fastboot erase data
fastboot erase cache
All successful , however after that still boot to original OS.
Tried installing Stock ROM/Firmware using - RSDLite , it seems to install copied the files on reboot .. same old OS ..
i can goto fastboot , i can run adb shell command ( but its read only system ).
i am really stuck here wondering if you guys help / much appreciated.

Factory reset yes , however after device reboot it does nothing comes back t same faulty OS.

You may try to flash rom again but be very careful with unlocked bootloader. It's strange that you cannot boot into custom recovery holding volume down and power after you flashed it.

Tried flashing , in safe mode no luck either , any other suggestions?

You probably left your USB plugged in after you flashed Recovery and that set you back to Stock Recovery.....unplug the usb....
Hmm sounds like symptons that my dad's old Nook color experienced being stuck in read only mode. Tried everything but it was stuck in that read only state no matter what I did. Hopefully that is not the case for your phone. Have you tried house of moto or other compatible utility to restore back to stock?
this happened a few times on an older faulty version of twrp. until someone can figure out how to remove the write only from fastboot, there is no utility or script that will fix it.
no one was able to figure it out before, unfortunately, your phone is likely toast.
Just saw this thread and it sounds exactly like what just started happening to my phone yesterday. I was unlocked, and rooted, with stock VZW (US) ROM. All was fine. Then started getting app error popups like you state and tried using TWRP recovery to install a backup I had made and it appears to work only if I DO NOT check the 'data' box. When I try and wipe data, it errors out as well. I am worried something caused it to get stuck in read only mode like one of the last comments in this thread and the phone is toast, but not just.
If you find anything or figure it out please let me know, I'll do the same....

I am guessing this might be the issue and I am experiencing it as well. However, I have no idea how that could have happened in the first place. I never tried to do anything with TWRP until after the phone began acting funny with all apps crashing (pop ups stating that) and the phone essentially becoming unusable.
When I am in TWRP and do wipes (factory reset, or advanced wipes) it seems to wipe everything fine, even the data partition. However, when I try and write to that partition it fails (i.e. restoring a backup including the data partition). If I don't include the data partition the restore works, but the phone is still crap.
Does this sound like the issue you mention below (data is stuck on read only and there is no way around it)?? If so, anyone in the market for a decent looking paperweight??

Well it looks like I was able to get through some of it. I followed the instructions here: http://www.droidrzr.com/index.php/topic/49699-1834614xt926-ota/. I did the extra step of going to stock 183 (unrooted) and then flashed the update in that and the phone is working fine. I am already unlocked (have been for long time), but not rooted at the moment. I believe I can root now (being that I am unlocked) but haven't tackled that yet. At the very least I have a phone that works for my upcoming business trip.....
BTW - the TWRP I had installed has been used on my phone for a long time (8-12 months??) so not sure why that version all of a sudden would have caused the issue, but at this point it doesn't appear to have been that issue (with data locked in read only mode).

I believe my LG G3's internal storage is corrupt or damaged. I cant boot in the OS and I can only boot in TWRP. If I try to format /data or any partition, it will fail and say it "Cant mount /data" and then it says the same for the cache. I tried LG FlashTool but it fails at 6 percent and says "Erase command failed". Trying to boot in to OS will fail and go to black screen warning that there is a kernel panic and say "not syncing:ext4-fs panic from previous error, remount as RO"
Would there be any script or file that would be able to load on to a micro sd card, and then work in TWRP to fix my partitions?

This sounds like exactly what happened to me. See my thread here. I tried absolutely everything included parted tools within TWRP. The internal eMMC flash was indeed dead. I was lucky enough to get AT&T to warranty mine without noticing TWRP.
I went to an AT&T official repair center, and told them I had already tried every tool including the LG Flash tool (which they apparently don't even have access to) to show them I knew what I was doing. The guy watched it bootloop multiple times pulling the battery a couple times in between and then said they would warranty it.
At that point, they ship you a refurb with just the phone body (no battery and no backplate) and have you send just the body of yours back . They don't check software once they receive if you read the fine print, only physical/water damage or tampering. Hopefully all that was a little helpful.
You may have already tried this, but maybe take a look at the Change or Repair file systems in TWRP? That way you can force a reformat of the sdcard, rather than just the standard data wipe.
This happened to me today I will appreciate anyone that can help me, can I fix this using TWRP repair filesystem?

And it exactly fails too at 4%. It auto reboots on twrp sometimes.

at first: I tryed to post my problem in the CAT s61 section. But i got the Message that i do not have enough posts/points to do so. It was suggested by the forum to post my thread in Q&A.
I updated my CAT S61 via OTA, same way as the last updates. I am not sure to what version the update was. I had no problems or issues with the phone before.
Very shortly after booting up my phone crashes. I have to boot it up again and get that message every time:

I shared a picture of the phone via GDrive. There is no option to upload pictures.
I can't find any useful help on the CAT-Phones site. The FAQ on the Site is mostly stuff for Android beginners. No PC tools or anything.
I don't want to lose my data and wipe my phone as suggested on the phone Message....
What can i do?

Same problem here, CAT S61, no idea if I had an update but today keeps rebooting to Android Recovery Screen,
If I power off and on I can get back into the phone with an annoying "Toolbox has stopped" error, CAT FAQ says to force stop but before I can get that far it reboots again.
Phone is charged.
Did you fix the problem?

Hey Simon, yes i got an email response from the cat support. I was on a buisness-trip, sorry for my late reply. I fixed it.
You have to open the app details of the toolbox app. force stoo the app, and under a sub-option in the apo info delete its cache and reboot.
If your cat s61 is in the restart-loop you can force stop ist by pressing power and vol down simultanously for dunno 8-13 seconds.
You have to be real quick. i needed like six attemps. but it worked for me.
No dataloss at all.
Thanks Dominus, I did find those instructions before your reply and tried it multiple times, but always before I could complete the instructions the phone would restart again.
I ended up doing a hard reset, not a pleasant experience, lost everything of course, photos were all in the cloud fortunately.
Phone is now operating normally again, I hope this isn't going to be a regular thing, I will certainly persevere with your instructions if it does.

Hello!! A friend of mine have a CAT S61 that after update won't boot and displays this error "Your deviced is corrupt. It cant be trusted and will not boot" but he didn't tried to root it or something like that.
And when I told him to get into recovery and wipe data he got this error of "no such file or directory.."
Do you know how to solve this error or do yo know where I can get the Stock Rom or OTA to flash and try? Ty

It seems they tried flashing something that was not meant for their device and now it has corrupted the system partition or somehow wiped out the operating system.
You will need to flash the full stock firmware to solve this. Can you boot the device into its flash mode? Download mode, fastboot mode or some similar flashing mode.
Do a Google search for:
"Return to stock (your model number)"

He says he didn't tried to flash anything. But I think the only way to solve this will be flash the stock Rom. The problem is that we didn't found the stock Rom and CAT didn't answered our emails.
